Typical Pile Driving Crane - the clamp failed and the beam struck the employee

A construction worker died Friday in a work-zone accident on Main Street in the Village of Hammondsport, Steuben County, NY.
Sharon L. Biesecker, 50, of Wheeler, and an employee of Economy Paving of Cortland, was found dead at the scene, New York State Police said.

"A large steel beam broke free from a clamp and when it broke free, it struck her in the back and hit her in the head," lead investigator on the case Jeremy Hagadone said.
State Police in Bath responded to the scene on the Main Street bridge around 12:04 p.m., near the Hammondsport Central School District building.
Hagadone called the situation a "freak accident."
Workers were using a crane to hammer steel beams into the ground. The investigator called the equipment faulty. The Occupational Safety and Health Administration are continuing to investigate.
A portion of the street was closed several hours.
The state police were assisted by the Village of Hammondsport Police and Fire Department, Occupational Safety and Health Administration and New York State Department of Transportation.
