High Potential Near
Miss – Update - Order Respecting
Dangerous Operation on HMDC GBS Slab
October 14, 2014
On Friday, October 10, 2014 the C-NLOPB’s Chief Safety Officer
(CSO) issued an Order Respecting Dangerous Operation for work involving
personnel working on top of the Hibernia Gravity Base Structure (GBS) slab,
following a High Potential Near Miss that occurred on September 14, 2014.
The gravity base structure at the deepwater site consists of a
105.5m concrete caisson, constructed using high-strength concrete, reinforced
with steel rods and pre-stressed tendons.
As a result of the investigation report submitted by HMDC, it
is the opinion of the CSO that continuation of this work has the potential to
result in serious bodily injury.
No further work activity is to occur involving personnel
working on top of the Hibernia GBS slab until measures/barriers for the safe
conduct of such work are in place to the satisfaction of the CSO.
Positioning the topsides on the gravity base structure.
