Firefighters from multiple departments are on the scene of a four-alarm fire in Kingston.



The fire broke out early Saturday morning at Northland Forest Products on Depot Road, a business that processes lumber shipments from across the country, officials said.

"Luckily, no one was inside any of the buildings," said Graham Pellerin, Kingston Fire Cheif.

Two buildings are involved and police estimate that millions of dollars worth of damage has been done, but the owner said that most of their inventory was saved.

"The most important thing is that no one was hurt," said Jameson French, President of Northland Forest Products. "The fire departments have done a great job to save the buildings that they could save and we're grateful for that."

To our Valued customers, suppliers and friends,

Early Saturday morning, a major fire did serious damage to our Kingston , NH lumberyard. We lost our pre-dryer, thermal modification oven, secondary inspection line, stacker, 3 forklifts and countless pieces of equipment and tools.

With the valiant efforts of the Kingston, NH and neighboring Fire Departments it appears our boiler and Kilns have been saved and no one was seriously injured.
With much hard work ahead of us we look to the future, knowing that we will emerge from this a stronger company. Although our loss is great, we have our most valuable resource, our employees. With their strong work ethic and determination, we will work to meet our customer’s needs, remain a valuable source and purchaser of hardwood lumber and continue to be a great place to work.

Ninety percent of our inventory is intact, we hope to have our kilns back online soon, and our primary inspection line should be running early this week. The lumberyard will be open as usual on Monday morning, shipping orders and starting the cleanup.


We would like to thank everyone for their kind words, prayers, and offers of help. As we sort through this we will undoubtedly have need for all of them.

2 NEBRASKA FIREFIGHTERS OVERCOME BY HEAT AT FIRE

August 4, 2016



Two volunteer firefighters were overcome by the heat early Thursday as they fought a fire in the Ponca Hills neighborhood in Omaha, Nebraska.


Several volunteer firefighting units and Omaha firefighters battled the early morning fire at a house on an acreage near U.S. Highway 75 and Calhoun Road north of Interstate 680.


One firefighter from Ponca Hills was seriously injured and taken to Immanuel Medical Center. Another volunteer firefighter, from Irvington, was critically injured and also taken to Immanuel.


Ponca Hills Fire Chief Joel Sacks said the injured firefighters were overcome by heat exhaustion while battling the blaze in hot and humid conditions. He said the Ponca Hills firefighter was released from Immanuel on Thursday morning and the Irvington firefighter was released a little later Thursday.


Sacks said firefighters were forced to run their hoses 100 to 200 feet from the nearest hydrant to the driveway’s entrance and then another 800 feet along the driveway to the home.


“That slowed things down a bit,’’ he said. “Logistically, it’s a nightmare to lay all that stuff out.’’


The blaze, which began in a pickup truck in the driveway before spreading to the home’s garage and then to the house, was reported at 1:20 a.m. Sacks said the cause was ruled as accidental.


One person and a couple of dogs were home when the fire began, he said. All escaped unharmed.


Volunteer units from Ponca Hills, Boys Town, Bennington, Irvington and Fort Calhoun responded to the blaze.

2 firefighters hospitalized after blaze north of Omaha

OMAHA, Neb. -- Two fire fighters are recovering from heat exhaustion after battling a house fire in Ponca Hills early Thursday morning.

The fire was reported in a home near Ponca Road and North 48th Street around 1:30 a.m.

Authorities say the cause was accidental. It apparently began in a vehicle, spread through the garage and then worked its way up to a second floor bedroom.

No one in the home was injured. A dog was also rescued with the help of a little oxygen. The dog, Trouble, ran back into the home after crews had cleared the scene.

The two fire fighters suffering from heat exhaustion are expected to be okay. One of them was pulled from the home and placed on a stretcher.

One fire fighter from Ponca Hills had been released from the hospital as of 7 a.m. The second, from Irvington fire, was doing better and staying the hospital a little longer to recuperate.

Fire fighters from Irvington, Bennington, Fort Calhoun, Boys Town and Omaha were called in to assist battle the fire in the hot, humid conditions. Crews went through over 75 bottles of water.
Along with the weather they were also challenged by distance. The fire hydrant was at the bottom of a hill leaving crews to pull close to 900 feet of hose.


Ponca Hills Fire Chief Joel Sacks called it a, "difficult location, extremely long driveway."

They had the fire largely under control within approximately 45 minutes. The homeowners said they have family nearby to stay with. The home is a partial loss.

Volunteer firefighter Lt. Jack Rose of the Mount Marion Fire Department died Dec. 19 from injuries sustained fighting a fire at 11 Fel Qui Road in the Town of Saugerties. Based on investigative reports, the events unfolded this way, though not all times are exact:

6:58 p.m. — The initial 911 call comes in

7:18 p.m. — Rose joined two other firefighters and took a hose into the first floor of the house to put out the flames there. That done, he and a second firefighter discovered additional fire in the basement. Rose and two firefighters grab a hose and re-enter the basement.

7:30-7:38 p.m. — A firefighter notices Rose is missing. He goes outside and asks if Rose has come out. No, he is told. Other firefighters open additional doors and windows, feeding oxygen to the fire and hiking its intensity.

7:38 p.m. — Centerville Fire Department calls a MAYDAY, meaning a firefighter is missing.

7:38-7:47 p.m. — Numerous firefighters search for Rose. Searchers find Rose near the center of the basement. When asked if he is OK, Rose said, “No, get me out.” He is taken outside and life-saving procedures started.

Rose is later declared dead at Kingston hospital. A medical examiner rules the cause of death as “asphyxia due to inhalation of heated products of combustion.” The official time of death: 9:35 p.m. Dec. 19.

New York state has issued 14 violations to two fire departments regarding the Dec. 19 line-of-duty death of Mount Marion volunteer firefighter Lt. Jack Rose.
The Public Employee Safety and Health Bureau of the state Department of Labor issued the violations in early May. The Times Herald-Record obtained the reports on the two departments through a request under the Freedom of Information Law. 


Two key findings in the reports point to possible contributing factors in Rose's death: 


  • Rose, who was 19, was using someone else’s self-contained breathing apparatus pack — equipment he had not been fitted for and not trained to use — that he grabbed off the truck of a different fire department at the scene of the fire that night.
  • Rose and the firefighters with him inside and outside the house failed to stay in contact. 

The citations are based on a lengthy investigation, including interviews with those at the scene, done by both PESH investigators and Saugerties police. Safety and health officials issued eight violations to Rose’s Mount Marion Fire Department and six violations to the Centerville Fire Department. 


The fire at 11 Fel Qui Road in Saugerties was in the Centerville Fire District and, as such, Centerville was in charge at the scene. It sent one truck and seven firefighters. Mount Marion responded with two trucks and six firefighters, including Rose, under mutual aid, according to the reports. 


Fire chiefs of the Mount Marion and Centerville departments did not respond to interview requests. 


According to the reports, Rose had second-degree burns on his nose and his lower forehead. “Lt. Rose may have removed his mask in order to obtain these burns,” the Mount Marion investigation report says. 


The Centerville investigation report says the burns “are likely from a heated object striking him in the face, such as an open door.” Or, the report said, his face could have struck something as he was being taken from the burning building. 


The mask was not the one specifically fitted to Rose. Neither report explains where Rose’s Scott brand SCBA was that night. Both reports quote another Mount Marion firefighter saying that he and Rose “grabbed two SCBA packs off the Centerville fire truck.” He also commented to police that “firefighters, on occasion, will grab SCBA packs from another department’s fire trucks.”
The air pack from the Centerville truck was a different brand. 


Rose got separated from the other firefighters. “Why this happened is unclear,” the Mount Marion report says. “Additionally, due to the presence of thick smoke and high-temperature conditions, it is believed that Lt. Rose became disoriented as to his position relative to the exit door and thus began searching for an exit.”
Rose, the reports state, did not tell the two firefighters with him on the hose line in the basement that he was leaving. They did not keep track of him either, nor did those outside the basement who were supposed to monitor firefighters inside the house. 


Both department are required to correct the violations, though the time frame for those actions was not detailed in the reports. Failure to correct the violations could result in fines of up to $200 a day per violation

MARINE – SERVICE STANDARDS

I.                    Acknowledgement

A.                  Always by e-mail unless otherwise requested.

II.                  Contact

A.                  Immediate contact with vessel owner and boatyard to arrange inspection.

B.                  Contact by telephone and e-mail confirming date and time of inspection.

C.                  Copy of e-mail and notes in file.

III.                Investigation

A.                  Identify vessel by the Hull Identification Number (HIN) and compare to the policy.  Obtain hours on the vessel and serial numbers on the engine when possible.  Comment on how the vessel is maintained.

B.                  Inspect vessel within the scope of the claim, i.e., if mechanical, make sure you have the proper personnel present if you do not have the qualifications.  The client will direct as to what they want covered.

C.                  Inspect damage both inside and out as damage may transfer.  Damage often in a fiberglass vessel will show on both the hull and the interior, i.e., bottom damage on the outside may be minor, but the interior could have major damage such as stringer separation or cracking.

D.                 Scene inspections should be identified either by marina locations or the longitude and latitude coordinates on the water.

E.                  Statement from the captain and crew when instructed.  Identify official personnel present, i.e., Department of Natural Resources (DNR) or marine police.  Obtain report number and where it can be obtained.

F.                   Photograph all sides of the vessel, along with good photographs of the damage or the accident scene.

G.                 Obtain the name of the boatyard where the Insured wants the vessel repaired and try to get an agreed scope and price for the repairs.  All yards vary in their pricing.

IV.                Experts

A.                  Experts are frequently used on marine claims, depending on the fields of expertise needed.  Information can be found on the Internet or by using the directories provided by the surveying associations SAMS and NAMS.

V.                  Reporting

A.                  First report is always a captioned report on stationary and copied and e-mailed.

1.         Assignment

2.         Coverage state policy number and dates and coverage to be involved or coverage questions to be discussed and researched.

3.         Vessel identification, HIN number to police and full description of the vessel.  Is it well-maintained, including name, length, width, power and serial numbers?

4.         Description of Incident – Full description from the vessel captain or operator.  Full description of the damage involved to insured vessel or claimant’s vessel.  Identify all parties involved and separate captions for them - Insured, Claimant, Witnesses, DNR or Marine Police.

5.         Repairs scope of the damage with open area, if any, and agreed price with the owners repair yard.

VI.                Conclusion

A.                  Further Handling – List all work needed to complete the investigation or the completion of the damage report.

VII.              Billing

A.         Unless prior agreement was made, billing is done on a time and expense basis to be sent in with the final report.

Metropolitan  is aware that contaminated and non-representative samples can cause lengthy delays for Oil Majors and Traders - both after loading and prior to discharging.

That is why METROPOLITAN has experience with specialized closed sampling equipment which can eradicate the time lost due to the necessity of re-sampling or re-analyzing owing to erroneous contamination claims on sensitive and light cargoes. The system ensures that samples are not contaminated with particulates from the vessels vapor locks.

Unlike many vessels' sampling equipment used for obtaining cargo tank samples; the equipment we can employ draws representative running, spot or dead bottom samples on oil tankers via their vapor lock system.

In today's work place, with advancing safety and environmental considerations, the system eradicates the need for "open sampling" and enables accurate representative samples to be drawn.

The closed sampler ensures that no gases are expelled from a vessel tanks during sampling, essential when dealing with high H2S cargoes.

Our aviation adjusters are capable of handling hull claims and liability claims on all types of aircraft and aviation facilities. We maintain a strong roster of experienced aviation engineering and mechanical specialists. Our aviation adjusters are top notch, with A&P licensed professionals (with real world experience) and FAA Airframe Engineers acting as our estimators, former Commercial Pilots & Airline Transport Pilots acting as our C&O consultants (and adjusters in some cases) and Inspection Authorization professionals for review and quality assurance of our estimates. 

 We maintain a solid relationship with aviation MRO's and FBO's throughout the US, and can perform accurate adjusting with Accelerated results.

Claims Management System


Many clients mean many claims management or tracking systems. Our adjusters and examiners are familiar with most "out of the box" style management systems, as well as a number of customized systems. For those clients that may not have their own system developed, we can offer a web based customizable system, tailored to the specific needs of the client.

Our claims management system is constantly under development in an effort to keep abreast of new advances in technology and deliver streamlined access and intuitive document retrieval. 

This system not only allows secure access to relevant documents, reports and at-a-glance status updates, but also allows adjusters and examiners to collaborate in real-time via video, voice or text to generate the perfect end product. 

 Currently, for catastrophes, we utilize the FileTrac CMS.

Our file handling process generally far exceeds the standards for file handling issued by our client carriers. Below is a brief description of the typical step-by-step process that has become our personal standard for handling your files:



  • Claim is received via email, fax, online or by telephone

  1. Claim is entered into our CMS
  2. Claim is assigned to adjuster, who is notified via telephone and email
  3. Acknowledgment is sent to client

  • Adjuster Handling

  1. Insured is contacted on the same business day of assignment
  2. Loss is inspected within 48 hours
  3. Estimate is completed using the Xactimate estimating platform
  4. Report is submitted for internal review within 5 days

  • Internal Review

  1. Each and every file is reviewed by a staff examiner intimately familiar with the policies and procedures of your company
  2. File revisions are performed in house to ensure speedy delivery to the client 
  3. *Complete report is submitted to the client within 7 days
    • In the event of a large loss or directions from the client to secure an agreed cost or bids from a contractor or emergency mitigation service, a first report will be submitted within 5 days.

  • Revisions & Re-Inspections

  1. Any carrier requested file revisions are completed the day they are requested
  2. The client will not be charged any additional fees if a re-inspection is required as a result of items that were overlooked by the field adjuster (hidden damages do not apply).
