Lack of Communication ID’d as Probable Cause of 2015 Houston Ship Channel Collision involving the chemical tanker Carla Maersk

A pilot’s inability to respond appropriately to hydrodynamic forces and a lack of communication was found to be the probable cause between a bulk carrier and a Maersk chemical tanker in the Houston Ship Channel in March 2015, the National Transportation Safety Board has determined.

The collision between the 623-foot bulk carrier Conti Peridot and 600-foot tanker Carla Maersk occurred on a foggy morning on March 9, 2015 and resulted in the release of approximately 88,200 gallons of methyl tert-butyl ether (MTBE) from the Carla Maersk. The incident cause the Houston Ship Channel to be closed for three days.

The NTSB investigation into the collision determined that the probable cause of the collision was the inability of the pilot on the Conti Peridot to respond appropriately to hydrodynamic forces after meeting another vessel during restricted visibility, and his lack of communication with other vessels about this handling difficulty controlling his vessel, the NTSB said during a public meeting in Washington, D.C. on Tuesday.

“The collision occurred in restricted visibility after the pilot on the Conti Peridot was unable to control the heading fluctuations that the bulk carrier was experiencing during the transit. As a result, the Conti Peridot crossed the channel into the path of the Carla Maersk,” the NTSB said in a synopsis of the report.

Contributing to the circumstances that resulted in the collision was the inadequate bridge resource management between the master and the pilot on the Conti Peridot. The NTSB noted on twitter that the collision was the fifth accident the NTSB has investigated since 2011 involving large vessels in or near the Houston Ship Channel.

“No ships sank and no lives were lost in this collision, but the release of more than 88,000 gallons of MTBE into the waterway, which resulted in the surrounding communities sheltering in place immediately following the release of the hazardous materials, underscores the severity of this accident,” said NTSB Chairman Christopher Hart. “The Houston Ship Channel also supports one of our nation’s busiest seaports, hosting more than 60 ship and 350 barge movements each day. Accidents that disrupt navigation of this vital waterway can have significant impacts, not only to our environment, but also to our economy. Effective bridge resource management can make the difference between a near-miss and a tragic accident, and this is particularly true in the narrow and congested Houston Ship Channel.”

The NTSB identified three safety issues in the accident including insufficient pilot communications, inadequate bridge resource management, and the lack of predetermined ship movement strategies during restricted visibility in the Houston Ship Channel.

The NTSB issued three new safety recommendations to Bremer Bereederungsgesellschaft mbH & Co., (the operating company of the Conti Peridot), the Houston Pilots Association, and the Lone Star Harbor Safety Committee, based upon the findings of the investigation.

A Panamanian-flagged tanker averted potential disaster overnight after the vessel developed a crack in its hull in the Arabian Gulf off India.

The Indian Navy says it was called Wednesday night to provide assistance to the MT Infinity 1 which was taking on water and had developed a dangerous list approximately 20 miles from Goa.

The INS Trikand along with the Coast Guard cutter Amal arrived on scene and were able to transfer emergency dewatering equipment to the vessel. With the flooding under control, the Trikand escorted the ship to Karwar where was it was safely anchored.

A teenager has died after falling down an elevator shaft inside an abandoned building in Toledo, OH

TOLEDO, Ohio (WTVG) - A teenager has died after falling down an elevator shaft.



It happened around midnight Wednesday inside an abandoned building at the corner of Detroit and Phillips.

Police said seven teenagers were inside the building when 16-year-old Joshua Sorrell fell down an elevator shaft.

Three of those teens stayed with Sorrell, performing CPR until police arrived.

When officers arrived they found the teen laying on the ground outside of the vacant building.

Sorrell was later taken to a nearby hospital where he died.

They are also trying to determine if the fall was accidental or intentional. The investigation is on-going.

TOLEDO (13abc Action News) - A 29-year-old man from Sylvania is dead after a motorcycle accident overnight on Reynolds Road.

Police say he was headed north on Reynolds when he lost control of his motorcycle and jumped the curb in front of Dunbar Mechanical Inc. at 2806 N. Reynolds

It happened around 2:40AM. The man died from his injuries. Troopers say he was traveling alongside another person on another motorcycle and they were both going at a high rate of speed.

Seven Coast Guard units work together to rescue 3 people and a dog aboard disabled sailboat in Oregon

NORTH BEND, Ore. – Coast Guard air and boat crews worked together to assist three people and a dog aboard a 51-foot sailing vessel that was disabled and adrift about 45 miles west of Siuslaw River, Wednesday.

The crew of the Coast Guard Cutter Cuttyhunk, a 110-foot patrol boat homeported in Port Angeles, Wash., is towing the sailing vessel Rogue Scholarship toward Reedsport and will transfer the tow to the crew of a 47-foot motor life boat from Coast Guard Station Umpqua River.

Watchstanders at Coast Guard Sector North Bend initially received an uncorrelated distress call at 9:30 a.m. Forty five minutes later, watchstanders received a digital selected call distress notification with a location about three miles west of the observed location of the first notification. Aircrews launched aboard two MH-65 Dolphin helicopters from Coast Guard Air Station North Bend and Coast Guard Air Facility Newport, a boatcrew launched from Coast Guard Station Siuslaw River aboard an MLB, the crew of the Cuttyhunk diverted from patrolling nearby and a C-27J medium range surveillance aircraft from Coast Guard Air Station Sacramento to search for possible mariners in distress.

The master of the vessel was able to contact the C-27J aircrew and reported that all three people on board were in good condition, but the vessel had a torn sail, fouled propeller and had been adrift for three days. A Dolphin aircrew arrived on scene and deployed a rescue swimmer with a VHF-FM radio. The crew of the Cuttyhunk arrived on scene and placed the sailboat in tow and is expected to arrive near Reedsport at about 9 p.m.

“This was a complex case involving assets from different parts of the country,” said Wesley Trull, the operations unit controller at Sector North Bend. “Our air, boat and cutter crews were able to work together to get these mariners in distress the help they needed.”

Weather on scene was reported to be 4-to-6 foot seas with 10-to-15 knot winds and a water temperature of 57 degrees.
