Pilot killed, after Iraqi Air Force F-16 crashed during night training mission five miles east of Douglas Municipal Airport, AZ

An Iraqi F-16 assigned to the 162nd Wing, Arizona ANG, crashed sometime around 20.00h today, five miles east of Douglas Municipal Airport, while partaking in a night training mission. 

The Iraqi pilot, Brig. Gen. Rafid Mohammed Hassan, is believed to have been killed in this mishap.

The crash of the F-16 occurred near Douglas close to the border between Arizona and New Mexico. The jet was the inaugural F-16D block 52 #1601 (RB-01) for the Iraq Air Force which first took flight on May 2nd, 2014.

The crash sparked a brush fire that was burning next to a high-pressure gas line. Emergency personnel from the Fire Department was pulled back from the area due to the proximity of the gas line. Homes in the vicinity were being evacuated.

Operating out of Tucson National Airport, the 162nd Fighter Wing is the largest Air National Guard fighter wing in the USA, and has been training pilots since 1969. I also operates the Air Force's international F-16 training academy. The Wing averages 17,000 flying hours per year. Arizona has excellent year-round flying weather. Fewer than 3 percent of all scheduled sorties are canceled due to bad weather conditions.

The Iraq government ordered 36 F-16s and while delivery of them is on hold, the pilots who will fly them have been learning the ropes of the aircraft with the 162nd FW since 2012.

On Thursday afternoon , around 17:40 hours , the sea below the hotel Resnik , crashed sport plane with three crew ! It is one pilot lost his life , and a subsequent inspection interior aircraft firefighters have found another body . The third pilot in turn severely injured taken to hospital in Firule . Unofficial sources told us to be in a coma, suffered a fracture lubalnog vertebrae and numerous fractures of the ribs .

At the scene were kaštelanski firefighters and ambulances and fire the ship arrives and DVD Kastel Gomilica . As we found out from eyewitnesses , avoiding the even greater tragedy since the plane for only a few meters missed many swimmers who were at the time of the fall located on the beach .

Sports aircraft Lake Buccaneer LA - 4-200 ( type of aircraft that does not have a black box AN) , is owned by European Coastal Airlines and is not intended for commercial flights in the air was only four minutes ; pilot training , he learned to manage aircraft , after which the country due to unknown reasons hit the canopy hotel Resnik and crashed into the sea . After the initial shock , bathers rushed to help the victims , but unfortunately two of them could not be saved .

The police reported that the accident Port Authority , and the Agency for accident investigation in air, sea and rail transport , which is the court sent the chief investigator Goran Strmečki , which is expected to reveal the causes and circumstances of the crash .

All nine people on board a small float plane that was taking cruise ship passengers on a scenic flight around the far-Southeast Alaska community of Ketchikan died from injuries sustained in a crash into a cliff Thursday afternoon.

Eight passengers of Holland America Line's MS Westerdam were participating in a shore excursion operated by Ketchikan-based Promech Air, when shortly before 1 p.m. something went wrong. 

Federal Aviation Administration cameras show that the cloud ceiling was low, and winds were whipping around Misty Fjords, where the DeHavilland DHC-3T Turbine Otter crashed into a granite rock face near Ella Lake.

A similar aircraft -- a turbine-powered DeHavilland DHC-3ST Otter -- was at the center of a July 2013 crash in Soldotna that killed a pilot and two families who were visiting from South Carolina.

Longtime Alaska Sen. Ted Stevens also died in a DHC-3T Otter.

In the Thursday crash, a Temsco helicopter pilot initially spotted the downed aircraft approximately 800 feet above the Ella Lake, according to an Alaska State Troopers dispatch.

Jerry Kiffer, on-scene commander for Ketchikan Volunteer Rescue, said it took two hours to get to the scene of the crash because of the inclement weather. 

Rescuers were dropped a few hundred yards from the crash site and climbed the rest of the way to the site, which he described as steep and covered in moss.

The aircraft appears to have hit the base of a 50-foot rock cliff and then come to a rest on the 60-degree base of the slope, Kiffer said.

"Certainly, we've done this many times before," Kiffer said of the attempted rescue. "No different than many they've been on. It's important we get the job done and get it done quickly."

Chris John of the Ketchikan Volunteer Rescue Squad said 13 volunteers with the organization went out to search the scene of the crash Thursday in two helicopters and two floatplanes. He described conditions as “very turbulent” with winds around 25 mph.

“The conditions were really steep,” John said. “It was a bad crash site.”

National Transportation Safety Board spokesman Clint Johnson said it is unclear if weather played a role in the crash, but that will be investigated, and for now the rain and wind is hampering efforts to investigate.

"Obviously we're going to have to play the weather by ear and hopefully get this done sometime tomorrow," Johnson said late Thursday by phone.

The cruise line said in a statement that the ship was visiting Ketchikan as part of a seven-day Inside Passage voyage which began June 20.

On Promech Air's website, a floatplane trip to Misty Fjords was advertised for $229 per person and billed as a "once in a lifetime trip."

"Whether you are visiting Ketchikan by cruise ship, vacationing in Ketchikan, touching base with friends and family nearby or just looking for a day trip out of Ketchikan while on business, this tour is a great way to see this unique corner of Alaska," the company wrote of the 75-minute trip, which includes a 10-minute water landing on a lake or fjord.

While the ship's departure was temporarily delayed according to passengers and family members who contacted Channel 2 News, the MS Westerdam could be seen on closed circuit cameras leaving moments before 7 p.m.

"We are incredibly distressed by this situation, and our thoughts and prayers are with those on board the plane and their families," Holland America officials wrote in a news release. "Holland America Line is extending its full support to traveling companions of the guests involved."

Promech declined to comment on the incident when reached by phone by a Channel 2 News reporter, but the company later sent a news release.

"There is nothing I can say that can alleviate the pain and overwhelming sense of loss that we and the loved ones of those affected are feeling," wrote Marcus Sessoms, the company's president. "At this moment, all of us share the pain and anguish of this terrible event."

Troopers said victims of the family will not be identified until next-of-kin have been notified, a process that can take hours or days.

Notes from the Field: Update: Silicosis Mortality — United States, 1999–2013

Silicosis is a potentially fatal but preventable occupational lung disease caused by inhaling respirable crystalline silica (silica) (1). 

 Chronic silicosis, the most common form, occurs after exposure to relatively low silica concentrations for >10 years. Accelerated silicosis occurs after 5–10 years of exposure to higher silica levels, and acute silicosis can occur after only weeks or months of exposure to extremely high silica concentrations (1). New national mortality data for silicosis have become available since a previous report on silicosis surveillance was published earlier this year (2). 

CDC reviewed multiple cause-of-death mortality files from the National Center for Health Statistics to analyze deaths from silicosis (International Classification of Diseases, 10th Revision diagnosis code J62: a pneumoconiosis due to dust containing silica) reported during 1999–2013. 

Each record lists one underlying cause of death (the disease or injury that initiated the chain of events that led directly and inevitably to death), and up to 20 contributing causes of death (other significant conditions contributing to death but not resulting in underlying cause). 

Available death certificates from 35 states were reviewed for the period 2004–2006 to identify occupations associated with silicosis among decedents aged 15–44 years. Results indicate that despite substantial progress in eliminating silicosis, silicosis deaths continue to occur. Of particular concern are silicosis deaths in young adults (aged 15–44 years). 

These young deaths likely reflect higher exposures than those causing chronic silicosis mortality in older persons, some of sufficient magnitude to cause severe disease and death after relatively short periods of exposure. A total of 12 such deaths occurred during 2011–2013, with nine that had silicosis listed as the underlying cause of death.

During 1999–2013, a total of 2,065 decedents had silicosis listed as the underlying or as a contributing cause of death (1,122 [54.3%] decedents had silicosis listed as the underlying cause of death) (Table). 

The annual number of silicosis deaths declined 40% from 185 in 1999 to 111 in 2013 (p-value for trend <0.001), but the decline appears to have leveled off during 2010–2013. The lowest number of silicosis deaths (88) occurred in 2011. Higher numbers of deaths occurred in 2012 (103) and 2013 (111), but remained within the 95% confidence interval predicted by the first-order autoregressive linear regression model used to evaluate trends for 1999–2013. 

Among all silicosis deaths, 47 (2.3%) decedents were aged 15–44 years; of these, 34 (72.3%) had silicosis coded as the underlying cause of death (Table). The annual number of silicosis deaths in persons aged 15–44 years varied and was 4, 0, and 8 in 2011, 2012, and 2013, respectively.

Death certificate review identified 62 silicosis deaths, accounting for 13.7% of the 451 reported silicosis deaths during 2004–2006. Of 39 (62.9%) decedents with silicosis listed as the underlying cause of death, three were aged 15–44 years. 

Entries on death certificates of these young decedents related to industry and occupation were classified* as miscellaneous nonmetallic mineral product manufacturing (stationary engineers and boiler operators), construction (brickmasons and blockmasons), and cut stone and stone product manufacturing (crushing, grinding, and polishing machine setters, operators, and tenders). These industries and occupations are well-known for their association with exposure to crystalline silica (1).

Silicosis mortality in the United States has declined over time (2,3). The continuing occurrence of silicosis deaths in young adults and reports of new occupations and tasks that place workers at risk for silicosis, including fabricators and installers of quartz-containing engineered stone products and workers employed to extract natural gas by hydraulic fracturing (47), underscore the need for strengthening efforts to limit workplace exposure to crystalline silica. 

Effective silicosis prevention strategies for employers are available from the Occupational Safety and Health Administration and CDC's National Institute for Occupational Safety and Health.§ State health departments can strengthen silicosis prevention efforts by identifying silicosis cases through review of state morbidity and mortality data and by investigating the circumstances surrounding silicosis cases.
