Charleston, Wa., Airport, Insurer at Odds Over Runway Extension $26 million Collapse. Designer Triad Engineering and contractor Cast and Baker on the hook for the failure and damages.

The failure in March of a geosynthetically reinforced runway extension at Charleston, W.Va.’s Yeager International Airport has triggered a lengthy dispute among the airport’s insurance carrier, AIG Aero, the designer Triad Engineering and contractor Cast and Baker over the liability for damages, currently estimated at $16 million to $26 million.

“It’s been 55 days since the incident, and we have yet to see an adjuster from any party here at the site,” Michael Plante, Yeager airport spokesman, said on May 12.

At a special airport board meeting this month, Plante said Yeager refinanced its debt and began to settle claims from displaced residents because of delays in negotiations between the insurance companies representing the airport and its contractors. “AIG has told us that the airport is not at fault, and we like hearing that, but we want the community made whole,” Plante said.

At least one contractor, SE Clearing and Hydroseeding, was hired to perform recovery work on the condition they would be paid when the insurance is settled, Plante told ENR.

Hailed as a geotechnical marvel of modern engineering—the highest geotechnically reinforced slope in the U.S.—upon completion in 2008, it collapsed on March 12, forcing the evacuation of over 100 residents. Several homes down slope were destroyed and a creek flooded.



Tencate, a leading global geosynthetic materials company that supplied a specialized mesh for the $25-million project, said the runway apron won an Award of Excellence in the geosynthetic projects category of the 2007 International Achievement Awards from Industrial Fabrics Association International.

Christine Gerard, IFAI, said the competition is judged by industry experts on creativity, function, design uniqueness and complexity.

It proved its efficacy in 2010, when the runway extension was credited with saving 34 people after a plane over ran the runway on an aborted takeoff.
Completed in 1947, Yeager is sited on a man-made plateau atop rugged, mountainous terrain. New Federal Aviation Administration regulations in the early 2000s, however, threatened Yeager’s continued operation.

Required improvements included adding 500 ft of emergency-stop apron for Runway 5, which meant working on a mountainside that slopes down steeply more than 300 ft to the Elk and Kanawha rivers.

“The entire slope did not fail,” Plante says. The next steps are to deal with the collapsed slope and issue a request for qualifications to hire a contractor to restore the extension to a capacity that meets FAA regulations, he said.

“I do believe the failure of this slope will have a major impact on the future of geotechnical engineering,” Koerner says. “It will cause designers to be much more conservative in their consideration of drainage and other hydraulic factors, which appear, at this point, to be the cause of the failure. It’s been distressed. They’ve known for sometime it was distressed.”

“We received two to three inches of rain in the days before the collapse, and a state of emergency for flooding was declared by the governor,” Plante says. The airport does not know if that contributed to it, but there was a significant weather event at the time.

Rick Valentine, Triad’s consultant, told ENR he could not share any of the findings from his investigation into the cause of the slip.

Fine-grain soil products available as fill material on similar projects also will receive more scrutiny from designers in the future, Koerner says. The slope was filled with 1.5 million cu yards of dirt, Tencate says.

John Lustombo, Tencate, said stormwater detention was not part of the company’s work on Yeager airport.

Triad Engineering and Cast and Baker did not return ENR’s request for comment.

OSHA, NIOSH release new toolkit to better protect hospital workers from transmissible diseases

WASHINGTON - The Occupational Safety and Health Administration and the National Institute for Occupational Safety and Health today released the Hospital Respiratory Protection Toolkit*, a resource for health care employers to use to protect hospital staff from respiratory hazards.

Respirators are used to protect against exposures to airborne transmissible infectious diseases as well as chemicals and certain drugs that may be used in healthcare settings.  OSHA’s Respiratory Protection Standard requires that health care employers establish and maintain a respiratory protection program in workplaces where workers may be exposed to respiratory hazards.

“Hospitals are one of the most hazardous places to work,” said Assistant Secretary of Labor for Occupational Safety and Health Dr. David Michaels. “One of the ways that we can protect workers in a health care setting is by providing employers with the resources needed to ensure a safe workplace. This toolkit will help protect those workers who dedicate their lives to caring for others.”

“Appropriate respiratory protection is a vital line of defense against airborne hazards hospital workers might face on the job,” said NIOSH Director John Howard, M.D. “This toolkit is an important resource to help health care employers ensure their workers are out of harm’s way when it comes to respiratory hazards.”

The toolkit covers respirator use, existing public health guidance on respirator use during exposure to infectious diseases, hazard assessment, the development of a hospital respiratory protection program, and additional resources and references on hospital respiratory protection programs. Appendix D is an editable document that each hospital can customize to meet its specific needs.

To supplement the toolkit, The Joint Commission, an accrediting body for more than 20,500 health care organizations and programs in the United States, developed an educational monograph, Implementing Hospital Respiratory Protection Programs: Strategies from the Field, to assist hospitals in implementing respiratory protection programs. The monograph, produced in collaboration with NIOSH’s National Personal Protective Technology Laboratory, identifies common implementation challenges, provides specific examples of innovative strategies from healthcare organizations and examines the role of leadership, quality improvement, fit testing and training challenges, and program evaluation.

“Respiratory protection programs enhance safety for both workers and patients, but there are many common challenges associated with their implementation,” said Ana McKee, M.D., executive vice president and chief medical officer, The Joint Commission. “We hope that by showcasing the innovative and effective strategies used by health care organizations across the country to overcome some of these challenges, hospitals can learn from one another as they implement and improve their own respiratory protection programs.”

NIOSH is the Federal agency that conducts research and makes recommendations for preventing work-related injuries and illnesses. It was created under the Occupational Safety and Health Act of 1970 and is part of the U.S. Centers for Disease Control and Prevention. OSHA and the Coordinating Committee for Automotive Repair renew alliance to protect workers in the car repair industry

WASHINGTON - The Occupational Safety and Health Administration has renewed its alliance with the Coordinating Committee for Automotive Repair. The alliance will provide CCAR members and others with training and resources to help protect the safety and health of workers in the car repair industry, as well as information on workers’ rights and employer responsibilities under the OSH Act.

“Workers in automobile repair shops are exposed to a variety of chemical and physical hazards,” said Assistant Secretary of Labor for Occupational Safety and Health Dr. David Michaels. “Our renewed alliance will continue to focus on training and education about the hazards workers face in their jobs and the necessary actions that employers must take to keep them safe.”

Through the alliance, OSHA and CCAR will develop training for automotive lift safety; working with absorbed glass mat batteries and automotive air conditioning refrigerant, with an emphasis on the hazards of contaminated refrigerant.

CCAR, established in 1994, is a nonprofit organization that provides safety, pollution prevention and HazMat training, along with educational and environmental best practices for the global motor vehicle industry. The alliance agreement has been renewed for five years.

Through its Alliance Program, OSHA works with unions, consulates, trade and professional organizations, faith- and community-based organizations, businesses and educational institutions to prevent workplace fatalities, injuries and illnesses. The purpose of each alliance is to develop compliance assistance tools and resources; share information about agency initiatives, resources, and programs with workers and employers; and educate workers and employers about their rights and responsibilities. Alliance Program participants do not receive exemptions from OSHA inspections or any other enforcement benefits.

CONSTRUCTION CRANE ACCIDENT IN CALIFORNIA KILLED NEVADA MAN

CRESTMORE, CALIFORNIA

A construction crane accident in Crestmore killed the 63-year-old Nevada man who was operating the machine, say San Bernardino County coroner's officials.

Judge W. Lightfoot died at the scene when the 75-ton crane toppled about 3:50 p.m. Monday, May 11, in an equipment yard along the 11400 block of South Cactus Avenue. He was a resident of Henderson, Nev.

The crane had been scheduled to travel to a construction job on Tuesday, fire officials said. Lightfoot was helping to check the machine and ready it for the trip, they said.

The crane actually fell on him. He was pinned inside the operator's cab.
The 75-ton machine landed on its side.

As evening neared, the crews of a fire engine, a ladder truck and a heavy rescue unit were making arrangements to raise the crane so that the victim's body could be removed.

It’s unclear how Lightfoot became pinned under the large piece of equipment.
An autopsy will be conducted to determine the cause of death.

ALLEGHENY VALLEY RAILROAD FREIGHT TRAIN DERAILS IN HAZELWOOD, PA; NO INJURIES OR SPILLS AS 11 CARS LEAVE THE TRACKS

HAZELWOOD, ALLEGHENY COUNTY, PA

Eleven cars of a long freight train derailed this morning in Hazelwood, Allegheny County emergency officials said.

The incident was reported about 10 a.m. near the 4100 block of Second Avenue and Irvine Street, officials said. Those streets were closed to outbound traffic for a while after the derailment, but both have now reopened.

John Poister, state Department of Environmental Protection spokesman, said there were no injuries and the derailed cars were empty.

Some of the cars were on their sides, and their wheel sets separated from the car bodies.

Allegheny Valley Railroad confirmed the derailed train belonged to the company but had no further comment.

The derailed part of the train, immediately behind its five locomotives,  included mostly covered and open hopper cars. The train also was pulling dozens of tanker cars, but none of them derailed.

A man who lives on Irvine Street about 100 yards from the derailment said he heard the usual banging noises that trains in the area make, but then “I heard the engines slam down [their power] real quick and I knew something was going on.”

The man, who would not give his name, said he frequently sees trains hauling crude oil, corrosive acid, chlorine and diesel fuel on the Allegheny Valley tracks.

”We’re just lucky that’s not what got pulled off the rails“ today, he said.
