NTSB AND FAA INVESTIGATION CONTINUES AT LINCOLN COUNTY, TN CESSNA 182P SKYLANE DEADLY PLANE CRASH SITE

For a second day, investigators with the NTSB and FAA sifted through a pond on a farm in Mulberry where a plane crashed on Sunday.

The biggest task was pulling the plane from the pond to send off for testing. Investigators hope it will shed light on the crash that killed 46-year-old Linda Bauman.

"It's very surreal," said Zachary Colescott, manager of the Winchester Municipal Airport. "Linda is one of the last pilots I would have ever thought something like this would happen to."

Bauman took off from the Winchester Municipal Airport around 2:30 p.m. Sunday. Pilots who knew her said they are just now coming to grips with what happened.

"This incident happened on Sunday afternoon and we didn't find out about it until Monday morning, so that hurts quite a bit," Colescott said. "Personally, thinking that there was something we could have done in the meantime if we had found out earlier."

Airport officials said filing a flight plan isn't required by the FAA in nice weather, but it could have helped in this crash.

Now, airport officials want to make changes to improve communication for others.

"We will go back ... and talk about procedures and some things we could've done differently and maybe put together a checklist to make things happen a little bit quicker," Colescott said.

Airport staff remembered the mother of two as an experienced pilot who was always jovial but careful.

"Her personality, just always a big smile," Colescott said. "It was always like she was having fun. But I think behind that was a tone of seriousness and she really respected what she was doing in flying the airplane.

Investigators have released the name of a female pilot killed in a single-engine plane crash in rural Lincoln County Monday morning.  

Linda Bauman, 46, of Belvidere, Tennessee was killed Sunday afternoon when the small plane she was piloting crashed in a pond on a farm along Steelman Road, off Highway 50 in the community of Mulberry. The farm owners were not home Sunday and did not find the wreckage until Monday morning. 

The debris field at the scene indicates that the plane hit a tree before plunging into the pond. A cow at the farm was apparently killed in the crash, according to News Channel 5 out of Nashville.

The TV station reports that Bauman's Cessna 182 crashed around 2 p.m. 

Sunday, though the cause of the crash was not yet clear. Bauman was reported missing Sunday night. 

Lincoln County Sheriff Murray Blackwelder told AL.com's news partner, WHNT News 19, that the pond will be drained to retrieve the plane. Though the pond is just five feet deep, the sheriff is reluctant to send in divers because of sharp metal shrapnel in the wreckage.

EAST MOLINE, ILLINOIS HAZARDOUS MATERIAL TRAIN DERAILMENT STILL UNDER INVESTIGATION

Railroad officials are still trying to determine why a pair of tanker cars  came off  the tracks in East Moline on Monday. 

The  cars -- part of a 16-car Canadian Pacific freight train loaded with flammable chemicals -- derailed at slow speed about 9:30 a.m. Monday in the 1000 block of 12th Avenue, East Moline, said Canadian Pacific spokesman Andy Cummings. There were no spills or injuries, and the cars did not overturn. The accident site was cleared by the afternoon.

How the accident occurred remained under investigation Tuesday, he said. He could not provide information on any damage the cars or engine may have sustained in the accident. 

BNSF owns the tracks.

BNSF spokesman Andy Williams said the derailment did not cause significant damage to the tracks and that the stretch of railway where the accident occurred was operational as of Tuesday afternoon.

According to hazardous material numbers on various tanker cars, contents included acetone, anhydrous ammonia and isopropanol, a highly flammable chemical that “may form an explosive mixture when mixed with air,” according to cameochemicals.noaa.gov.

One of the derailed cars was carrying isopropanol, and the other contained nonflammable chemicals.

We got lucky this time and there was no threat to public safety, East Moline Fire Department engineer Dale Shrum said Monday. "It does happen, and it didn't pose any threats to us at this time.  There is always a next time, and we may not be so lucky then.”

A town in North Dakota has been evacuated by authorities after an oil train derailed on Wednesday morning, according to multiple reports.

Valley News Live reports the Wells County Sheriff’s Office has confirmed a BNSF train has left the tracks near Heimdal, with witnesses reporting black smoke in the area.

An image of the crash by Jennifer Willis published on Facebook shows tankers have left the track and caught fire.

Wells County emergency manager Tammy Roehrich told the Fargo Forum that the train derailed around 7:30 a.m., setting six oil tanker cars on fire.

KHND reports motorists have been told to steer clear of the area by emergency officials, while KFGO has confirmed that Heimdal is in the process of being evacuated, and that no injuries are reported.

Heimdal is a small town located between Harvey and New Rockford, and has a population of just 27, according to the 2010 census.

SurveyMonkey CEO David Goldberg died of severe head trauma in an exercise accident in the Mexican resort town of Punta Mita, a Mexican state official said Monday.

Goldberg, husband of Facebook executive Sheryl Sandberg, was found lying next to a treadmill on Friday at a resort in Punta Mita near Puerto Vallarta on Mexico's Pacific coast.

The official said he left his room at about 4 p.m. to exercise, and family members went to look for him after he didn't return. He was found at about 6:30 p.m. in a gymnasium lying by a treadmill in a pool of blood, with a blow to the lower back of his head. He apparently had slipped on the treadmill and hit the machine, said the official, who spoke on condition of anonymity because the person was not authorized to speak to the press.

The official said Goldberg still had vital signs when he was discovered, but later died at a hospital in Nuevo Vallarta. The official said the cause of death was severe head trauma and hypovolemic shock, or bleeding.

The official said the accident took place at the Four Seasons Resort Punta Mita and that the family had checked in on April 30.

But the Four Seasons issued a statement saying the incident did not happen on any of its properties and that David Goldberg was not registered as a guest in any of the rooms, villas or residences.

Serious injuries involving exercise equipment, treadmills in particular, are among the most common that emergency rooms see, according to data collected by the U.S. Consumer Product Safety Commission.

Hospital emergency departments across the USA treated some 24,400 treadmill related injuries last year, said CPSC spokeswoman Kim Dulic.

The agency counted 30 reported deaths associated with treadmills from 2003 to 2012.

Almost 460,000 people were sent to the hospital in 2012 for injuries related to exercise equipment, according to the agency's data. The vast majority—nearly 428,000 were treated and released for their injuries—but about 32,000 were hospitalized or were dead on arrival.

Treadmill injuries included broken bones, abrasions, rectal bleeding and people developing chest pain while working out on the machines, according to a review of the CPSC data base system.

The death of SurveyMonkey CEO David Goldberg, who authorities says suffered severe head trauma after slipping from a treadmill, comes nearly six years after the 4-year-old daughter of former heavyweight boxing champ Mike Tyson died in a treadmill accident at her home in Phoenix Ariz. 

She had been strangled by a cord connected to the machine.
