THE CAUSES OF CRANE FAILURE ARE UNIVERSAL: Loader crane overturned after failing to use suitable mats or cribbing

A loader crane overturned yesterday on a job site in Israel after failing to use suitable mats or cribbing.
Few details are available of the incident, other than the images which have been sent in by an observer, however it appears the crane, an Effer loader crane mounted on a four axle truck/trailer, overturned when one of its outriggers slipped off of the rough cribbing onto the soft ground. At the same time the jack seems to have buckled away from the outrigger beam.
It is not clear if this failure contributed to the slippage or was purely a result of it happening. As far as we know no one was injured in the incident, we will update if we receive any more information.
