INVESTIGATION FINDINGS
On January 10, 2015 at approximately 1100 hours, a finger
injury occurred at Anadarko Petroleum's East Breaks 602 A, Nansen SPAR, Lease
G-14205.
Two contract construction workers were installing a new
8" ANSI 900 isolation valve upstream of the total fluid dump valve on High
Pressure (HP) separator #1, MBD-1010.
The isolation valve flange was in close proximity (confined
area) to the HP separator vessel support.
This restricted the area that the Injured Party (IP) had to strike the
Cameron wrench/hammer wrench to tighten the flange bolts on the isolation valve
flange.
One construction worker (CW) held the hammer wrench with a
rope to secure the hammer wrench in place.
The IP attempted to strike the hammer wrench with the 2# hammer. The 2#
hammer deflected off the HP separator vessel support causing the IP's left
index finger to be smashed between the hammer handle and the hammer wrench.
The IP immediately reported to the facility Medic for
evaluation and first aid.
The Medic determined that the IP's left index finger was
broken and should be flown in for X-Rays and additional medical
evaluation. The IP remained in sickbay
until the field helicopter arrived at 1400 hours.
The IP was flown via the field helicopter to the ERA base in
Lake Charles, LA then transported to All Industrial Medical Services in Houma, LA for treatment.
The IP was diagnosed as having multiple
fractures to the second bone of his left index finger between the first and
second joint as well as being dislocated.
The IP underwent surgery to his left index finger on January
14, 2015. The IP remains under
Physician's care and was reassigned to ty pending full release.
LIST THE PROBABLE CAUSE(S) OF ACCIDENT:
The IP failed to recognize the hazards of working in a
confined area.
LIST THE CONTRIBUTING CAUSE(S) OF ACCIDENT:
The isolation valve bolts were in a confined area with
little room to strike the hammer wrench with the 2# hammer.
LIST THE ADDITIONAL INFORMATION:
The IP was flown via the field helicopter to the ERA base in
Lake Charles, LA then transported to All Industrial Medical Services in Houma,
LA for treatment. The IP was diagnosed as having multiple fractures to the
second bone of his left index finger between the first and second joint.
The IP had less than one year offshore experience.
Surgery was performed on January 14, 2015.
The IP remains under Physician's care.
The IP was reassigned to light duty pending full release.
