A worker was killed after a steel grate tipped over and pinned him at a concrete site in Lincoln, Nebraska




LINCOLN, Neb. --



A worker was killed after a steel grate tipped over and pinned him at a concrete site in Lincoln on Monday.




The call came into the industrial concrete site near 70th Street and Arbor Road around 11:30 a.m.

Lincoln Police said two employees were at the scene when a large steel piece of equipment, called grizzly bars, fell on top of one of one of them.

The other worker on scene called for help.

The worker who became trapped under the grizzly bars was pronounced dead at the scene, and police said heavy machinery was necessary for the recovery.

Police are still investigating what led up to the accident and said they will be on scene for several more hours.

LPD said OSHA is also on scene investigating.

NTSB issues preliminary report on BNSF oil train crash at Doon Iowa

WASHINGTON, D.C. — 


On June 22, 2018, about 4:35 a.m. central daylight time, southbound BNSF Railway (BNSF) freight train U‐CPGSAP‐059 derailed 33 jacketed DOT‐117R tank cars at milepost 165.3 on the BNSF Marshall Subdivision in Doon, Lyon County, Iowa. The train was being operated in a distributed power configuration with 2 head-end locomotives, 2 head-end buffer cars, 98 tank cars, 1 rear-end buffer car, and 1 trailing distributed power unit (DPU) locomotive loaded with about 2,426,325 gallons of UN1267 petroleum crude oil. Maximum authorized speed on the territory was 49 mph. The Federal Railroad Administration (FRA) determined the train speed was about 48 mph when the train encountered the emergency brake application. A total of 10 tank cars were breached, releasing about 230,000 gallons of crude oil. The area received 5 to 7 inches of rain during the 48 hours prior to the accident, washing out track and flooding a tributary of the Little Rock River and farm fields adjacent to the derailment location. (See figure 1.) Released crude oil reached the Little Rock River and prompted the evacuation of 18 to 20 people. No injuries were reported. A unified command consisting of BNSF, U.S. Environmental Protection Agency, Iowa Department of Natural Resources, and Lyon County, Iowa, was established to mitigate and recover the released crude oil.

ConocoPhillips Canada Marketing & Trading ULC originated the shipment from the Hardesty Terminal at Rosyth, Alberta, Canada, and its destination was the ConocoPhillips Company in Houston, Texas. The hazardous material released in this accident was identified as Surmont Mix A, a heavy crude oil and diluent mixture that exhibited an American Petroleum Institute (API) gravity of 19.9, initial boiling point of 115.0 degrees, flash point of -31 degrees, and vapor pressure of 4.7 psi. While these characteristics would have placed the material in Packing Group II (flash point less than 73 degrees and initial boiling point greater than 95 degrees), the shipper classified the material as Hazard Class 3, Packing Group I (highest degree of danger).

On July 10, 2018, National Transportation Safety Board (NTSB) investigators completed on-scene work in Doon, Iowa, where tank car wreckage was staged after the accident. Additional investigative work to examine parts removed from one of the tank cars is planned at the NTSB laboratories in Washington D.C.

The derailed tank cars were originally specification DOT-111A100W1 that were built to the Association of American Railroads CPC-1232 industry standard for tank cars ordered after October 1, 2011, for use in crude oil and ethanol service. Each tank car had about 29,000 gallons capacity and was equipped with a jacket, insulation, and full-height headshields. From 2016 through 2017, the tank cars were retrofitted to specification DOT-117R in accordance with Pipeline and Hazardous Materials Safety Administration (PHMSA) regulations, by replacing the bottom outlet valve operating handle with a disengaging mechanism designed to prevent unintended opening in accidents.[1] Preliminary findings from the examination revealed breaching damage in 10, or about one-third, of the derailed tank cars. Parties to the investigation include FRA, PHMSA, BNSF, The Greenbrier Companies, and Trinity Tank Car.

An estimated 230,000 gallons (870,619 liters) of crude oil spilled into floodwaters in the northwestern corner of Iowa following a train derailment, a railroad official said Saturday.
BNSF spokesman Andy Williams said 14 of 32 oil tanker cars just south of Doon in Lyon County leaked oil into surrounding floodwaters from the swollen Little Rock River. Williams had earlier said 33 oil cars had derailed.

Nearly half the spill – an estimated 100,000 gallons (378,530 liters) – had been contained with booms near the derailment site and an additional boom placed approximately 5 miles (8.05 kilometers) downstream, Williams said. Skimmers and vacuum trucks were being used to remove the oil. Crews will then use equipment to separate the oil from the water.
“In addition to focusing on the environmental recovery, ongoing monitoring is occurring for any potential conditions that could impact workers and the community and so far have found no levels of concern,” Williams said.

The train was carrying tar sands oil from Alberta, Canada, to Stroud, Oklahoma, for ConocoPhillips. ConocoPhillips spokesman Daren Beaudo said each tanker can hold more than 25,000 gallons (20,817 imperial gallons) of oil.

Beaudo said Saturday that the derailed oil cars were a model known as DOT117Rs, indicating they were newer or had been retrofitted to be safer and help prevent leaks in the event of an accident.

The derailment also caused concern downstream, including as far south as Omaha, Nebraska, about 150 miles (240 kilometers) from the derailment site. The spill reached the Rock River, which joins the Big Sioux River before merging into the Missouri River at Sioux City.

Omaha’s public water utility – Metropolitan Utilities District – said it was monitoring pumps it uses to pull drinking water from the Missouri River.

Rock Valley, Iowa, just southwest of the derailment, shut off its water wells within hours of the accident. It plans to drain and clean its wells and use a rural water system until testing shows its water is safe.





The pilot Scott Watson of Marina Del Rey, 55, was killed when a 1980 Beech A36TC Bonanza single-engine aircraft crashed into a field near Interstate 5 in Los Angeles' San Fernando Valley.

Federal investigators will comb through the wreckage Monday of a small plane that crashed into a Sylmar field, killing the pilot.

Scott Frazier Watson, 55, of Marina Del Rey, was killed when his plane crashed into a field right alongside the 5 Freeway in Sylmar Sunday afternoon. The father of three was pronounced dead at the scene.

Several motorists on the freeway reportedly stopped and ran down the freeway to try to help the pilot before emergency services arrived. Traffic on the northbound 5 and 405 freeways ground to a slow crawl because the crash could be seen from the freeway.



Watson’s daughter, Katie, said her father had his pilot’s license for about 15 to 20 years and that he was returning home to the Van Nuys Airport from a weekend trip.

Watson, who was called by relatives as a computer genius who worked his way up to become head of technology for Disney Engineering, had made a mayday call for help, but it’s not known what was wrong with the plane.

Officials from the National Transportation Safety Board and the FAA will be on the scene Monday to look through the wreckage of the plane.



For being such a genius, he did not know that flying single engine planes is very dangerous.  Why take such risks, buddy when you obviously have all the money in the world?

The pilot of a small plane who died after the aircraft crashed in the Sylmar area on Sunday afternoon has been identified by the L.A. County coroner as a 55-year-old man.

Scott Watson of Marina Del Rey was the pilot who crashed in a field near the northbound 5 Freeway and Roxford Street, authorities said.

Preliminary information indicates that the single-engine Beechcraft BE33 crashed under unknown circumstances in the field located about 5 miles east of Van Nuys Airport around 1:45 p.m., according to Ian Gregor, a spokesman for the Federal Aviation Administration.

The pilot, only identified as a man, was initially trapped in the wreckage but was later freed and declared dead at the scene, an LAFD alert stated.

Witnesses said it appeared that the pilot was trying to land along a nearby freeway just before crashing into the field.

Will Markgraf, who heard the deadly crash, said it sounded like fireworks going off or gunshots being fired. Once the plane was down, he said, several people ran over to the scene.

"There was a mass of people," he said. "There were about 30, 40 people just surrounding the crash site."

It appears that no one else was on board the plane, and no other injuries were reported, according to the Fire Department.

The FAA and National Transportation Safety Board will investigate the incident, Gregor said.

No other details were immediately available.

LOS ANGELES, CA (AP) —


Authorities say the pilot was killed when a small, single-engine aircraft crashed into a field near Interstate 5 in Los Angeles' San Fernando Valley.

Los Angeles Fire Department spokesman Brian Humphrey says the plane, a Beechcraft A320, went down shortly before 2 p.m. Sunday. Humphrey says the pilot, the only person on board, was killed.

No one on the ground was injured and no structures were damaged.

Cause of the crash was not immediately known and the pilot's identity was not released.

Humphrey says the National Transportation Safety Board will be investigating.

The plane went down in an open field in Los Angeles' Sylmar neighborhood, in the northeast corner of the San Fernando Valley.

The pilot was described as an adult male.

For the second time in three days, a seemingly endless wave of torrential rain is pounding parts of New Jersey on Monday, flooding streets and houses, stranding drivers and spawning a waterspout near Long Beach Island.

The hardest-hit area of the state during the past few hours has been Monmouth and Ocean counties, where as much as 5 inches of rain had fallen in several towns as of noon and at least one place -- Brick Township -- was drenched by nearly 8 inches. That’s almost two month's worth of rain in just a few hours.

Most of Monmouth and Ocean escaped the wrath of Mother Nature on Saturday, when torrential rain clobbered parts of Bergen, Essex and Passaic counties, sending walls of water from overflowing rivers and lakes flowing through streets, yards and parking lots.

A similar scene is playing out today in some Shore communities, with rain water pouring into homes and office buildings and water rescues being reported in the Greenbriar section of Brick Township.

As of early Monday afternoon, a whopping 7.83 inches of rain was reported in Brick, where township officials declared a state of emergency. Nearly 5.6 inches of rain was reported in Howell Township and almost 5 inches was registered in Wall Township.

Heavy flooding was reported along a three-block section of Spring Lake, and several flooded roads were closed in Neptune Township, emergency management officials noted on social media.

In Sea Girt, numerous roads were flooded and closed to traffic Monday morning, and some traffic signals were out, according to the weather service.

Among the other areas with huge rainfall totals are Berkeley Township in Ocean County, with 3.85 inches, Pennsauken in Camden County, with 3.63 inches, and Cedar Bridge in Ocean County, with 2.46 inches reported by the New Jersey Weather & Climate Network, based at Rutgers University.

In addition, weather radar estimated that 5 to 6 inches of rain had fallen in other parts of the Jersey Shore as of early Monday afternoon. Some areas of Salem County and parts of Lakewood in Ocean County got hit with 2 inches or rain in 1 hour or less.









Flash flood warnings, river overflowing


The torrential downpours prompted the National Weather Service to issue a series of flash flood warnings and flood advisories throughout the morning and early afternoon.

Among the alerts that remain active are these:

A flash flood warning for northeastern Ocean County and southeastern Monmouth County, effective through 3:30 p.m. Monday.

A flood warning for the Passaic River in Pine Brook, effective until further notice. At 12:30 p.m. Monday, the river had risen to 19.6 feet, which is a half-foot above flood stage. Minor flooding is occurring this afternoon, and moderate flooding is forecast by the National Weather Service, which says the river will continue rising to about 20 feet by Wednesday afternoon before starting to fall.

A flood advisory for Middlesex County, central Monmouth County and southeastern Somerset County, effective until 2:45 p.m. Monday.

Waterspout along Jersey Shore One of the storm cells moving through New Jersey on Monday morning was so intense that it spawned a large waterspout near Long Beach Island in Ocean County. The waterspout -- which is a type of tornado that forms over water without hitting land -- was spotted near Ship Bottom shortly before 7 a.m.

It was the second waterspout to form in Ocean County during the past five weeks

Suffolk County Police Third Squad Detectives are investigating a motor vehicle crash that killed two people and injured five others in Brentwood this afternoon.

Olvin Jimenez was driving a 1997 Toyota Corolla southbound on Crooked Hill Road when he lost control of the vehicle and crossed into the northbound lane, where the vehicle was struck by a 2005 Toyota RAV4 at approximately 1:38 p.m.

Both the driver of the Corolla, Olvin Jimenez, 39, and front seat passenger Erica Jimenez-Mejia, 39, of 224 Sherman Avenue in Brentwood, were pronounced dead at the scene by a physician assistant from the Office of the Suffolk County Medical Examiner.

Two children, ages two and ten, who were passengers in the back seat of the Corolla, were transported to Good Samaritan Hospital Medical Center in West Islip for treatment of serious injuries.

The driver of the RAV4, Allissiana Rose Licata, 21, and passengers Nicholas Licata, 17, and Emmanuel Licata, 73, all of Bay Shore were transported to Southside Hospital in Bay Shore for treatment of non-life-threatening injuries.

Both vehicles were impounded for a safety check. The investigation is continuing. Anyone with information about the crash is asked to call the Third Squad at 631-854-8352.

A husband and wife were the two people killed Sunday afternoon in a two-vehicle crash on Crooked Hill Road in Brentwood, Suffolk County police said early Monday.

Five other people, including their two children, were injured in the crash. There was no update on their condition Monday, police said.

Olvin Jimenez was driving a 1997 Toyota Corolla south on Crooked Hill Road, just south of the Long Island Expressway, when he lost control of the vehicle and crossed into oncoming traffic, police said. His vehicle struck a 2005 Toyota RAV4 with three people inside, police said.

Jimenez and his wife Erica Jimenez-Mejia, both 39 and from Brentwood, were pronounced dead at the scene, police said. Their two children, ages 2 and 10, who were in the backseat of their car were taken to Good Samaritan Hospital Medical Center in West Islip for treatment of serious injuries. The 2-year-old child was in an appropriate seat, the police said Monday.

The driver of the RAV4, Allissiana Rose Licata, 21, and passengers Nicholas Licata, 17, and Emmanuel Licata, 73 — all three from Bay Shore — were taken to Southside Hospital in Bay Shore for treatment of nonlife-threatening injuries.

The circumstaces of the crash were still under investigation on Monday afternoon, police officials said.

Two people were killed and five people -- including children -- were injured when a driver lost control and veered into oncoming traffic on a Long Island road this weekend.

Suffolk County police say the driver, 39-year-old Olvin Jimenez, died in the crash in Brentwood on Sunday afternoon. Also killed was the front-seat passenger, 39-year-old Erica Jimenez-Mejia.

The driver was at the wheel of a Toyota Corolla on Crooked Hill Road when he crossed into the opposite lane, slamming into a Toyota RAV4.

Two children in the back seat of the Corolla, ages 2 and 10, were being treated for injuries at Good Samaritan Hospital Medical Center. They remain in critical condition.

The driver of the RAV4, 21-year-old Allissiana Rose Licata, and two passengers, 17-year-old Nicholas Licata and 73-year-old Emmanuel Licata, suffered injuries.

The cause of the crash is under investigation.

Utah man, Duane Youd, crashes jet plane into own home hours after arrest for domestic violence

Police now say the crash of a small Cessna jet plane into a house overnight in Payson, Utah, was an intentional act.


According to a statement by Payson police Sgt. Noemi Sandoval, Duane Youd, was arrested on a domestic violence call at about 7:30 p.m. Sunday.

Four hours later, he was released from the Utah County Jail and returned home about midnight accompanied by a police officer to retrieve personal belongings from the residence.

At 2:30 a.m. a dual-engine Cessna Citation jet was heard flying “low and loud” over a Payson neighborhood. Witnesses told Gephardt Daily they were then stunned when the plane plunged from the sky, clipping the garage of one residence before exploding in flames in front of Youd’s own home.

Youd’s wife and a juvenile were inside the home at the time of the crash, but managed to escape uninjured.

Police say the Cessna 525 Citation was “borrowed” from a hanger at the Spanish Fork Airport.

According to FAA records obtained by Gephardt Daily, the plane was owned by VanCon, Inc. in Springville and Youd was named as the company’s corporate pilot.

A Cessna jet crashes into Payson home; residence engulfed in flame

One person is dead after a small Cessna jet plane crashed into a home in Payson, Utah early Monday morning.



First responders were called to 594 E. Canyon Road at about 2:40 a.m. with reports of an explosion and fire. When they arrived, they realized the home had been struck by a plane.

According to Payson Police officials, the deceased was found in the plane wreckage, and two people who were inside the home managed to escape without injury.

Payson resident Slade Buhler told Gephardt Daily he heard the jet roaring overhead just moments before the fiery crash.

“I had just taken my dog out. Came in, sat down, and I heard this huge plane come from the east and it was headed west. I hurried and ran outside to see what the hell that was going on and you could just hear how loud it was, how low it was. Then it kept going north, north, north, and then you could hear it swinging back.

“It came right over, just went right up there … and no lights. I could barely see what it was. All of a sudden he got about right there, the lights came on and I thought, ‘Oh, my God. He’s going down! He’s going down!’

“I thought he was going to hit the mountain, right there, and all of a sudden ‘boom’ — a huge explosion…. The noise … you just couldn’t believe the noise.”

Firefighters worked quickly to knock down the flames, which caused heavy damaged to the garage and the front of the home.

The twin-engine Cessna 525 Citation is registered to VanCon Holdings LLC, a Springville, Utah business. It was manufactured in 1995.
