PLAINFIELD, Ind. – The Indiana Department of Labor hit Amazon with a safety order and proposed fines totaling $28,000 after a worker died in September at a Plainfield fulfillment center.

The incident happened on Sept. 24 at the facility on South Perry Road when Phillip L. Terry, 59, was performing maintenance work on a forklift. The front lift fell on him, and Terry died from multiple crushing injuries.

The Hendricks County coroner ruled his death accidental.

The safety order cites four serious violations against Amazon, each carrying a proposed penalty of $7,000. The violations included inadequate training and failure to follow proper procedures.

The order gives Amazon until Dec. 1 to correct the problems.

Amazon has several options in response to the state’s proposed penalties. The company can pay the penalty as-is, ask for an informal review or formally contest the state’s findings.

PLAINFIELD, Ind.—The Hendricks County coroner released an update into the investigation of a fatal forklift accident at the Plainfield Amazon Fulfillment Center on Sunday afternoon.

According to the coroner, 59-year-old Phillip L. Terry of Indianapolis sustained multiple crushing injuries from the incident. The most significant injuries were to his head and his torso, including multiple skull fractures and injuries to his brain.

Toxicology and micros results are currently pending, and Terry’s cause and manner of death will be available in 4 to 5 weeks.

The accident happened Sunday afternoon at the Amazon facility at 800 Perry Road around 2:45 p.m.

Investigators say they are working to determine what caused the accident.

Since there was a death in the workplace, the state’s Occupational Safety and Health Administration (OSHA) will also conduct an investigation.

Plainfield fire and police officers were called to the Amazon Fulfillment Center at 800 Perry Road around 2:45 p.m.

On Monday, an autopsy was performed on the victim, 59-year-old Phillip L. Terry of Indianapolis. The results were not immediately available.

Investigators say they are working to determine what caused the accident.

Since there was a death in the workplace, the state’s Occupational Safety and Health Administration (OSHA) will also conduct an investigation.

Workers denounce Amazon for speed-ups after two die on the job

Two Amazon workers were killed on the job in the US as the company forces workers to speed up work to an unsafe and dangerous pace.

Fifty-nine-year-old Phillip “Lee” Terry of Indianapolis, Indiana was crushed by a forklift around 2:45 p.m. on September 24 while on the job at the plant outside Indianapolis. Twenty-eight-year-old Devan Shoemaker of Millerstown, who was employed by Amazon for seven years, was run over by a truck Sunday, September 19 around 5:15 p.m. while working at the Amazon warehouse in South Middletown Township, Pennsylvania.

Amazon workers at the warehouses where the deaths occurred condemned the corporation as responsible for Terry and Shoemaker’s deaths through work speed-ups aimed at maximizing corporate profits.

One worker remarked, “My employer Amazon talks about safety, but really emphasizes SPEED. Maybe this poor man was worried about his rate, and got a bit careless? Maybe not. In any event he’ll be immediately replaced by another warm body—that is guaranteed. In a few weeks we’ll be working mandatory overtime to ramp up for Christmas sales. Overtime plus very busy. Injury rate will also increase.”

Another former worker of a warehouse where a death occurred said, “Considering IND5’s [Amazon’s Plainfield Fulfillment Center] lack of commitment to safety, this is unsurprising. I hope all of my friends still working there are alright physically, mentally, and emotionally in the face of this tragedy.”

Other workers said the deaths were preventable: “This is just horrible...Someone said it was preventable....Yeah take your time to follow OSHA rules and guidelines before just putting ppl out there….I’ve seen so much in my 8 years...This is the worst at this location.”

Another said, “Too much lack of safety in most facilities. I have worked them for 19 years and have talked to many people and I feel I am qualified to speak on the subject. So sad for the family and co-workers.”

While paramedics and fire fighters responded to the reported workplace accident in Indiana and attempted to save Terry, he was ultimately pronounced dead at the scene. Following an autopsy, an initial report from the Hendricks County coroner indicated that Terry experienced multiple crushing injuries, the most serious of which occurring to his head and torso involving multiple skull fractures that caused traumatic injuries to the brain.

Amazon workers discussing the death on social media suggest that Terry’s death may have resulted from the forklift being in maintenance for repair when it fell on Terry as he was working below it to replace the hydraulic lines.

Terry, the Amazon worker killed in Indiana, is survived by his wife, son, and two grandchildren. He had graduated from Indiana University and previously worked as a supervisor of marketing. While it is unclear how long Terry was employed with Amazon, fulfillment center workers make an average of between $12.17-$13.42 an hour.

Devan Shoemaker, who died in Pennsylvania, had initially worked for Amazon as part of their “outbound team,” working to collect orders from the South Middletown warehouse for shipment. He later worked as a yard jockey, docking trucks and directing drivers back into the loading docks.

According to an investigation by Pennsylvania State Police, Shoemaker was killed when he was run over and crushed by a truck while he was working at the fulfillment center’s docking station. Shoemaker reportedly had been attempting to assist the truck’s driver with loosening the “kingpin” under the truck’s trailer which had gotten stuck. Emergency responders pronounced him dead at the scene, leaving behind a wife and young child.

According to OSHA’s online database, Shoemaker is one of two Amazon workers who have been killed at the Amazon Fulfillment Center in South Middleton since 2014. On June 1, 2014, 52-year-old Jody Rhoads was killed at the South Middleton fulfillment center after sustaining multiple traumatic injuries from being crushed between the seat of a pallet truck she was operating and storage shelves inside the fulfillment center.

While facts surrounding the events leading to Terry being crushed to death by the forklift have yet to be released, his death and those of Devan Shoemaker and other Amazon workers are not simply the result of isolated accidents.

Workplace deaths at Amazon are ultimately the result of the drive for higher and higher corporate profits. At Amazon’s warehouses and fulfillment centers, this takes the form of demanding a workforce that is often required to undertake exhaustive work, fulfilling orders at peak productivity for shifts as long as 11 hours, with mandatory overtime on a day’s notice. In addition, Amazon enforces speed-ups that force workers to frantically attempt to make their productivity requirements or face discipline or being terminated all while performing exhaustive physical labor. Such speed-ups, caused by the drive for profits, naturally result in dangerous working conditions that have resulted both in numerous workplace deaths and serious injuries.

As the Amazon deaths take place, local governments are attempting to outdo one another to give Amazon millions of dollars in taxpayer-financed credits and investment in efforts to court the company to choose their location as a second headquarters. Indianapolis is one of the many US cities whose officials are vying to outbid competing states to court Amazon with tax rebates and giveaways. Any serious investigation by local governments into Amazon’s long documented history of workplace deaths, injuries, and grueling working conditions in establishment circles would be seen as jeopardizing the bid for Amazon to locate their second headquarters in their respective areas. For this reason, the horrific deaths of Terry and Shoemaker will likely produce no calls for better regulation of the warehouses, paving the way for future deaths.

HOLIDAY, Fla. (WFLA) – Deputies have confirmed former MLB player Roy Halladay is dead after a small aircraft crash in the Gulf of Mexico.

During a news conference Tuesday afternoon, deputies said a recovery effort is underway. The Pasco County Marine Unit and Swiftwater Response Team are on the scene.

Investigators say the small plane is upside down in shallow water north of Bailey’s Bluff in Holiday.

They have not confirmed any survivors at this point.

Deputies would not confirm what kind of aircraft it was, only calling it a “small plane.”

News Channel 8 has learned the tail number is registered to an address that belongs to former MLB all-star Roy Halladay.

It is still under investigation who owns the plane and if that person was aboard the aircraft.

Investigators say they have not found a flight plan so they don’t know how many people were on the plane.

Obviously he was an inexperienced pilot and he end up crashing this brand new plane.  He should have listened to his wife.  Too late!

Former Philadelphia Phillies pitcher Roy Halladay was killed when the small aircraft he was in crashed into shallow waters in the Gulf of Mexico near Holiday, Florida. He was 40.

The crash happened shortly after noon on Tuesday.

Pasco County Sheriff Chris Nocco said during a news conference that Halladay's ICON A5 went down around noon off the coast of Florida. The sheriff's office marine unit responded to the downed plane and found Halladay's body. No survivors were found.

Police said they couldn't confirm if there were additional passengers on the plane or say where it was headed.

The Philadelphia Phillies released the following statement upon word of Halladay's death:

"We are numb over the very tragic news about Roy Halladay's untimely death. There are no words to describe the sadness that the entire Phillies family is feeling over the loss of one of the most respected human beings to ever play the game. It is with the heaviest of hearts that we pass along our condolences to Brandy, Ryan and Braden."

Halladay was an amateur pilot who often posted on social media about small planes. ICON aircraft had posted a video with Halladay trying out a new plane.

Roy tweeted frequently about his love for flying.

Harry Leroy Halladay III, affectionately known as "Doc," was traded by the Toronto Blue Jays to the Phillies in 2009. He was one of just six pitchers to win the Cy Young Award in both leagues.

Halladay pitched a perfect game on May 29, 2010, beating the Florida Marlins 1-0. In his first-ever postseason start later that season, Halladay threw the second no-hitter in MLB postseason history against the Cincinnati Reds in Game 1 of the NLDS.

Halladay retired in 2013, signing a one-day contract to retire as a member of the Blue Jays, where he spent the first 12 years of his career.

Other baseball players to die in plane crashes included Pittsburgh Pirates star Roberto Clemente in a relief mission from Puerto Rico traveling to earthquake victims in Nicaragua on New Year's Eve in 1972; New York Yankees catcher Thurman Munson piloting his own plane near his home in Canton, Ohio, in 1979; and Yankees pitcher Cory Lidle piloting his own plane in New York City in 2006.

SPEED KILLS, AGAIN! 17-year-old Sebastian Serrano and 19-year-old Daniel Castellano died after their high-speeding 2009 Ford Fusion crashed into a parked pick-up truck, then into a wall at a construction site in Staten Island, NY


Two teenage boys were killed in a single car crash on Staten Island Monday night.

Police say the car was driving at a high rate of speed northbound on Richmond Avenue in the Bulls Head section.

They were near the intersection on Signs Road when the driver lost control and crashed into a parked pick-up truck, then into a wall at a construction site.

The teens were taken to Richmond University Hospital where they were pronounced dead.


They have been identified as 17-year-old Sebastian Serrano and 19-year-old Daniel Castellano.

A teenage driver and his young passenger died in a gruesome Staten Island wreck Monday night, after he crashed into a parked car and slammed into a construction site.

The driver, 19-year-old Daniel Castellano, was heading north on Richmond Ave. behind the wheel of a 2009 Ford Fusion at about 8 p.m., when he hit a parked car near Signs Road in Bulls Head, cops said.

He then crashed into a pole and through a wall at a construction site, cops said.

The impact left both the driver and his passenger, Sebastian Serrano, 17, mortally wounded.

Medics took both to Richmond University Medical Center, but they couldn’t be saved.





"Someone came into my restaurant and said, 'Hey, help! Call 911!'" said the owner of the nearby Oriental Plaza restaurant, who only gave his first name, John. "When I came out all the cops were already here.”

The wreck happened less than a mile away from a crash that left an on-duty NYPD detective with a fractured skull Sunday night.

Det. Lisa Bergen, 50, is in stable condition after a minivan struck her as she and her partner crossed Richmond Ave. near Akron St. She suffered skull fractures, three fractured ribs, a fractured pelvis and a fractured vertebrae, police sources said.

The 31-year-old driver does not face charges.

Crash victim, 19, came back from Army 'ready to take on the world,' friend says.

STATEN ISLAND, N.Y. -- It was about 1 a.m. on Tuesday when Giovanni Balsamo learned it was a childhood friend who was one of two men who died in a fatal crash on Richmond Avenue in Bulls Head.

"As soon as I found out it was Danny..." said Balsamo, fighting through tears during a telephone interview Tuesday with the Advance. "I still can't believe he's gone."

Daniel Castellano, 19, of Bulls Head, was behind the wheel of a 2009 Ford Fusion that made impact with parked car, telephone pole and ultimately a construction site late Monday night on the 1900 block of Richmond Avenue, according to a NYPD spokesman.

Both Castellano and his passenger, Sebastian Serrano, 17, of Arden Heights, were pronounced dead on arrival at Richmond University Medical Center in West Brighton, according to a source with knowledge of the investigation.

Castellano, a graduate of Moore Catholic High School, had recently returned home from U.S. Army training at Fort Leonard Wood in Missouri, according to his Facebook page.   Unfortunately, this type of white trash are the ones who join the Army and the armed forces.  Reckless, bullies, aggressive, killers, etc.

According to a fundraiser on GoFundMe.com, Castellano was an honors student at one point and made it through boot camp.

"No matter what obstacles he had to over come he was a great kid," the fundraiser says.

Balsamo, who last spoke with Castellano about a year ago, said he had been meaning to catch up with his old friend, who by all accounts had come back from the military, "...with a clear mind, ready to take on the world."   Reckless drivers do not take over the world, buddy.  Only disciplined, law-abiding people do!

But he never got the chance to see it for himself.

"I got caught up with work and everything else," he said. "After this, I'm definitely gonna try harder to make time for friends. You never know."

Balsamo called off work Tuesday and headed to the scene, where he taped a farewell letter to a telephone pole.

"I wanted to take time out of my day to show how much I appreciated him as a friend and a person."

A man who answered the door at Castellano's home declined to comment on Tuesday.

Balsamo said he met Castellano they were 11 or 12 years old, and at one point spent nearly every day together. He described his friend as a dedicated older brother.

"If Danny heard his sister was getting picked on, he always was the first one trying to find out where it came from," he said.

A source told the Advance that speed is believed to be a factor in the crash, which police say remains under investigation.

A female friend, around the same age as Danny, wrote the following on Instagram in the hours following his death:


"I will always love you for your carefree spirit and constant joking and trying to make people laugh... safe passage on your travels until your final journey, may we meet again."   Of course this "carefree spirit" got him killed.  The Army was not able to correct the "carefree spirit" of this speedster (who is also a killer of his friend, Sebastian Serrano).

ALVA, Okla. (AP) —

Authorities in northwestern Oklahoma say a 61-year-old flight instructor is dead following the crash of a single-engine aircraft in Woods County.

The Oklahoma Highway Patrol says the crash occurred about 5:30 p.m. Saturday as the aircraft was approaching the Alva Municipal Airport. Troopers say a preliminary investigation indicates the aircraft experienced a mechanical failure during descent and that its landing gear struck some power lines. Authorities say the small aircraft struck some trees, plummeted to the ground and burst into flames.


Officials say the instructor, David Chael of Enid, was pronounced dead at the scene. A flight school student, 39-year-old Cory Washburn of Alva, was transported to a local hospital with undetermined injuries.The Federal Aviation Administration and the National Transportation Safety Board are investigating the cause of the fatal crash.

On Saturday, around 5:30 p.m., a private plane crashed in Alva, Oklahoma.

Officials said the Beachcraft Bonanza aircraft was approaching the north side of the airport when the cowl hatch opened.

Cory Washburn, 39, was piloting the aircraft at the time the cowl hatch opened; however, the pilot instructor, 61-year-old David Thomas Chael quickly took over, officials said.

The landing gear deployed and the front wheel struck power lines.

Officials say the aircraft then rolled over and came to a rest on its top.

Authorities say Chael and Washburn were exiting the plane when the aircraft exploded.

Chael was pronounced dead at the scene from massive injuries.

Washburn was rushed to the hospital in critical condition.

Officials say Washburn suffered burns over 80-percent of his body.

According to the Alva Review-Courier, Washburn had purchased the Beachcraft Bonanza last week.

He needed 15 hours of dual instruction before he could fly alone.

ENID, Okla. —

Dave Chael died over the weekend in a fiery plane crash in Alva.

Chael, 61, was a husband, father, pilot and friend who was loved by many.


“From all aspects, we know Dave was one of the best pilots there ever was,” Don Bryant said.

Chael was teaching Cory Washburn to fly when their plane crashed Saturday evening on Share Drive in Alva. The flight instructor died at the scene and Washburn survived.

“It’s been quite a shock,” Bryant said. “She just thought he was going out there to help someone learn to fly and then, of course, you get that phone call.”

Bryant is the interim pastor for Central Christian Church in Enid, where he said Chael was a very active member. Chael also was active with the Phillips University Legacy Foundation.

“Dave was the perfect mentor for our scholarship recipients,” said Kelly Coker, executive director of the Phillips University Legacy Foundation. “He was so insightful. He was always fun.”

“I know God’s got a lot of projects for you, and you’re at it right now,” friend Darrell Herndon said.

Plane Crash In Alva Leaves One Dead

  ALVA, Oklahoma -

Alva Police confirm a private plane has crashed north of the Share Medical Center, near Northwestern Oklahoma State University Ranger Field.

State troopers tell us a flight school student, 39-year-old Cory Washburn of Alva, and an instructor, 61-year-old David Chael of Enid,, were inbound to the Alva Municipal Airport when the small plane crashed around 5:30 this evening. Washburn suffered critical injuries and was rushed to the hospital. Chael was pronounced dead at the scene.

A witness described the plane as a ball of fire when it went down. Troopers tell us preliminary reports indicate a mechanical failure during the descent. The initial report from the Oklahoma Highway Patrol stated "(b)oth occupants were exiting the plane when it exploded."

“It's unclear what caused the malfunction with the aircraft,” said Trooper Justin Barney, "but the landing gear had been deployed and struck some power lines, causing it to go into some trees and then make contact with the ground.”

Another witness said the plane was flying low over the university football field just north of the crash site, then she saw smoke billowing up from behind a tree line. She says that smoke was flowing into Share Medical Center next to the crash site. The hospital had to evacuate some patients because power was knocked out but has since been restored.

Many of the homes in this neighborhood also lost power. Full power is expected to be restored around midnight.

Troopers are still notifying the victims’ next of kin so they have not yet released their names but say the student was taking private lessons from the pilot who was killed.

LAS CRUCES, NM - State officials are reporting four people were killed over the weekend in a plane crash near Hatch.

According to a release from New Mexico State Police, officers responded around 5:30 p.m. Sunday to the crash site, roughly 300 meters northwest of the Hatch airport. It's not clear when the crash happened.

Officers reported a fixed-wing, single engine Cessna aircraft crashed with four people inside. All were pronounced dead at the scene.

The four occupants have been identified as Anthony Deramus, 57, of El Paso; Christopher Howell, 33, of El Paso; Kanequa Chancellor, 41, of El Paso; and Malcolm Watkins, 25, of Waxahachie, Texas.

The cause of the crash is unknown, police report.

The Federal Aviation Administration was to arrive Monday to conduct an investigation, police said.

The incident marks the second fatal plane crash in Doña Ana County in the last month. On Oct. 12, Morris Doug Newton, 77, of Las Cruces and David Glenn Hancock, 67, of El Paso died in a plane crash roughly four miles northeast of the Las Cruces International Airport.

Hatch,NM/Dona Ana County (KTSM) - New Mexico State Police report four people, including three from El Paso, were killed in a plane crash just outside of Hatch, NM.

Around 5:30 p.m. on November 5, 2017, New Mexico State Police responded to a downed aircraft call in Dona Ana County, approximately 300 meters northwest of the Hatch airport.

Officers located the sight of the crash and reported a fixed-wing, single engine Cessna aircraft had crashed.

All four passengers were pronounced deceased by the Office of the Medical Investigator and have been identified as: 


Anthony Deramus (57) of El Paso, TX.
Christopher Howell (33) of El Paso, TX.
Kanequa Chancellor (41) of El Paso, TX.
Malcolm Watkins ( 25) of Waxahachie, TX.

The cause of the crash is still unknown at this time. The Federal Aviation Administration was contacted and will be arriving today to conduct an investigation into the cause and nature of the crash.

KTSM has confirmed the three El Pasoans killed in the crash were all members of Destiny Christian Church in Northeast El Paso. Chancellor served as College Youth Pastor and Deramus was a Deacon at the church. Chancellor was also a former UTEP women's basketball player in the late 90's.

Social media posts by Chancellor and Howell indicate that the group had borrowed a plane to fly Chancellor around the city and to Sparky's in Hatch for lunch as part of a birthday celebration for her.
