County worker dies after pickup truck crashes into a pond in Ohio

MASON, Ohio — 

Police say a man is dead after his pickup truck veered off a southwest Ohio road and crashed into a pond.

It happened just before 7:30 a.m. Thursday. Officials say 52-year-old Robert Butcher suffered a medical emergency while driving in Mason and crashed into a retention pond.

Rescue crews pulled Butcher from the submerged truck and rushed him to a hospital, where he was pronounced dead.

Officials say Butcher was a Warren County water and sewer worker.

No one else was in the vehicle.

The crash is under investigation.

TRIANGLE, Va. (WUSA9) -- One bicyclist was killed and two others were seriously injured after a cycling crash at the World Police and Fire Games Thursday afternoon.

The crash happened on the Loop, a 7.5 mile paved, windy road with hills at the Prince William Forest Park in Triangle, Va.

Prince William County police say the riders, all from Brazil, were transported to area hospitals for treatment. The two other riders remain in serious condition.

According to police, the crash was triggered when one of the riders sustained a front tire blowout, which caused a chain reaction with the other cyclists.

The deceased bicyclist was identified as 48-year-old Carlos Silva. The two other cyclists are a 44-year-old man and a 43-year-old man. Police have not released their names.

"The only thing he said about his life and his pleasure to be here that he loved what he do. He loved to be here. He loved to train and most of all, he loves his career, and his daughter," Alexandre Manzan, a colleague of Silva's said.

Riders familiar with the Loop say it's a tough course and can be dangerous for those who are not familiar with the road. The injured cyclists were coming down a steep, curvy hill over Quantico Creek.

The incident happened around noon, and the competition at the park ended shortly after.

The World Police and Fire Games are an athletic competition featuring law enforcement officers from all over the world. They released the following statement about the accident.

CHATTANOOGA, Tenn. (AP) - The truck driver who authorities say caused a highway accident outside of Chattanooga last week, killing six, was making his first run for the trucking company.

Officials from The National Transportation Safety Board told local media on Thursday that 39-year-old Ben Brewer had only started working with London, Ky.-based carrier Cool Runnings Express three days before Brewer's truck slammed into eight stopped cars.

The board's chief investigator Pete Kotowski says it could be 14 months before the investigation into the crash concludes.

The day before the June 25 accident, Brewer was cited for careless driving in Florida. He was supposed to undergo drug and alcohol tests afterward, but did not, officials said.

Kotowski says Brewer has been cooperative in interviews with investigators. No charges have been filed in the incident.

Police Identify 6 Killed in Crash on Tennessee Highway

Police have released the identities of six people killed on a Tennessee highway when a semi-truck hit eight vehicles that had stopped due to road construction.
Four of the victims of Thursday evening's crash were in one vehicle: 31-year-old Tiffany Watts and 50-year-old Sandra Anderson, both of Tennessee; along with two children, ages 10 or 11 and 8, both from California, Chattanooga Police Department spokesman Kyle Miller said in a news release.

Another victim, 37-year-old Brian Gallaher of Tennessee, was in a second vehicle, and 36-year-old Jason Ramos was in a third. Police listed Ramos as being from Georgia and Ohio.

Six people in four other vehicles received injuries that weren't life-threatening, and another four in one vehicle were uninjured, Miller said. The semi driver and a passenger in the truck also were uninjured. The crash happened on Interstate 75 northbound near the suburb of Ooltewah.

Miller said police are still investigating the cause. Meanwhile, the National Transportation Safety Board said it was sending a team to investigate.

The northbound lanes of I-75 reopened Friday morning, about 12 hours after the crash, when crews finished removing debris from the roadway.

Miller said it was the largest number of fatalities the police department has seen in a single crash in a decade. He said it was also the most vehicles the police department has seen in a single crash in that time period.

He said that area of I-75 has had only one other fatality in the past five years.

WELLINGTON, OHIO 

A railroad company employee fell six feet from the top of the train he was working on in Wellington and had to be hospitalized Thursday. 

About 2:30 p.m. Thursday the 63-year-old man and other CSX employees were working atop a slow-moving train in Wellington just west of Webster Road.

The workers were unloading rails and other equipment around the track when the man fell from the top of the train, according to Assistant Fire Chief Bill Brown. 

He injured his head in the fall and was found by first responders conscious but having difficulty breathing, Brown said. 

Wellington police and firefighters from Wellington Fire Department responded. The man was flown to MetroHealth Medical Center, Brown said. 

Firefighters did not have the name of the man or where he was from Thursday.

Grave Injury: Worker injured foot at Greenville, PA cemetery after gravestove fell on it

GREENVILLE, Pa. - 
 A worker at a Greenville gravestone company was rushed to the hospital Thursday after being injured at a local cemetery. 

According to the pastor at the St. Michael's Church on North High Street, the 51-year-old employee of Shelly Memorials was working in the St. Michael's cemetery when a gravestone fell on his foot. 

The worker was taken to UPMC-Greenville where his condition is unknown.
