WAYCROSS, Ga. -

A worker trapped in a piece of equipment at a Waycross asphalt plant on Wednesday morning has died, according to the Waycross Police Department.

By the time police and emergency crews arrived at the business on M Street about 8:30 a.m., coworkers had freed 35-year-old victim, Concepcion Zacatenco, of Douglas. Ware County Coroner Atha Lucas pronounced him dead.

Police said Avalos was killed while performing welding work on an asphalt silo at East Coast Asphalt. He was cutting away a piece of metal, safety equipment in place failed, according to police.

As a result, two clam shell doors that had been locked open fell shut and crushed Hernandez between them, police said.

His death remains under investigation by local agencies and the federal Occupational Safety and Health Administration.

Waycross police initially released the wrong name of the worker killed.
