Travis Layne Jordan,18, of Devers, Texas was killed in the head on collision.  He was most likely speeding, considering the massive damage he did to the Dodge pickup truck.

IVANHOE, TEXAS — 

An 18-year-old Devers man and his grandparents, as well as a 48-year-old man from Tyler were killed Monday afternoon in a wreck on U.S. Highway 69 in Tyler County.

The crash, which happened around 1:45 p.m. between Ivanhoe and Woodville, shut down the highway in both directions for more than four hours according to a news release from the Texas Department of Public Safety.

Troopers say a Ford F250 pickup driven by Travis Layne Jordan,18, of Devers, was traveling south bound when it crossed over into the north bound lanes and struck a Dodge pickup towing an RV and driven by Robert Alan Beard, 48, of Tyler.

Jordan and his grandparents, Harvey Lamar DuBose, 75, and Ann Jones DuBose, both of Hull who were riding with him were all killed in the wreck according to the release.
Robert Alan Beard, the driver of the Dodge, was also killed and a passenger in his truck, Leslie McCaa, 41, of Tyler, was transported to Christus St. Elizabeth Hospital in Beaumont with serious injuries according to the release.

The highway was reopened by a little after 7 p.m.

DPS have released the names of four people killed in an accident near Woodville, on Highway 69.

Beaumont media is reporting the crash, which happened around 1:45 p.m. between Ivanhoe and Woodville, shut down the highway in both directions for more than four hours, according to DPS.

Troopers say a Ford F250 pickup driven by Travis Layne Jordan,18, of Devers, was traveling south when it crossed over into the north lanes and hit a Dodge pickup towing an RV and driven by Robert Alan Beard, 48, of Tyler.

Jordan, and his grandparents, Harvey Lamar DuBose and his wife, Ann Jones DuBose, both of Hull, were killed in the wreck.

Beard was also killed. A passenger in his truck, Leslie McCaa, 41, of Tyler, was transported to a Beaumont hospital with serious injuries, according to the release.

The highway was reopened by a little after 7 p.m.

Troopers say they are still working to determine the factors that contributed to the crash.

Arizona man charged in fatal semi-truck crash

HUBBARD TOWNSHIP, OHIO — 

An Arizona man is facing several charges, including drunk driving, after he overturned his semi on Interstate 80 Monday evening, killing his passenger.

The Ohio State Highway Patrol said the crash occurred at 6:23 p.m. in the eastbound lane about 100 feet from the Pennsylvania border.

Ademayowa A. Saint, 46, of Glendale, Ariz., was driving a 2005 Freightliner when he went off the right side of the road, struck a ditch and several trees before jacknifing, a patrol report states. An unidentified female passenger in the sleeper berth — the portion of a semi between the driver’s cab and trailer — died in the crash. A report from the Trumbull County 911 Center states the woman was trapped inside for nearly two hours.

Saint was determined to be under the influence of alcohol and illegal drugs at the time of the crash, according to the patrol. Saint was expected to be taken to the Trumbull County Jail on charges of aggravated vehicular homicide, marijuana possession, open container and operating a commercial vehicle while under the influence. He had not been booked in as of 11:30 p.m. Monday. He was not injured.

The crash is still under investigation and the patrol is asking anyone who witnessed the crash to contact them at 330-898-2311. The patrol was assisted by the Eagle Joint Fire District, Hubbard Township Police and the Mercer County Sheriff’s Office.

HUBBARD, Ohio -

The Ohio State Highway Patrol says it believes an Arizona man was driving drunk when his semi-tractor trailer crashed along Interstate 80 in Hubbard, killing a woman who troopers believe was his fiancee.

Troopers say Ademayowa A. Saint, 46, of Glendale, Arizona was driving toward Pennsylvania at around 6:30 p.m. Monday when his truck went off the road, struck a ditch, and several trees before jackknifing.

Authorities have not released the name of the woman who was in the sleeper berth of the truck, but believe she was Saint's fiancee. The woman died in the crash. Saint was not injured.

The Ohio State Highway Patrol Crash Reconstruction Unit investigated the accident.

According to troopers, Saint was under the influence of alcohol and illegal narcotics at the time of the crash.

Saint was booked into the Trumbull County Jail on charges of aggravated vehicular homicide, possession of marijuana, having an open container, and operating a commercial vehicle while under the influence.

He is expected to be appear for a video arraignment on the charges Tuesday.

Eastbound I-80 was down to one lane after the crash but has since re-opened.

Any witnesses to the crash are asked to contact the Ohio State Highway Patrol Warren Post at 330-898-2311.

MACKINAC COUNTY, MI -- 

A Wisconsin pilot, who was an assistant district attorney in Milwaukee, crashed a plane into a Michigan lake on Friday, Aug. 31 -- and lost his life.


Ronald Dague, presumed dead

Officials with the Milwaukee County District Attorney's Office identified him as Ronald Dague, 64. Mackinac County sheriff's officials said he was from East Troy.

Below is a statement from John Chisholm -- Milwaukee County district attorney:

"Tragically, we learned that our colleague Assistant District Attorney Ronald S. Dague lost his life in an airplane accident last Friday night. Ron was a valued member of our General Crimes Unit and served with distinction in many capacities in the District Attorney’s Office since 1992.

On behalf of the District Attorney’s Office and all our colleagues in the criminal justice system, I extend my deepest condolences to Ron’s family and friends at this difficult time. Ron was a gifted trial attorney, a devoted student of the law, and a passionate defender of equity, fairness, and justice. His advocacy helped build a safer community for everyone living in Milwaukee County.

At a personal level, Ron was a devoted husband and father who deeply loved his family and his absence will be keenly felt. Please keep his wife and young daughter in your prayers. Dague was Milwaukee County's most experienced drunk driving prosecutor.

"The most complicated area of law and science that I've handled in my 23-year career," said Dague.

In 2013, he talked with the FOX6 Investigators about the failure to enforce ignition interlock devices.

"We have yet to be able to successfully prosecute even one case in Milwaukee County. So none," said Dague.

According to the Mackinac County Sheriff's Office, shortly after 9:45 p.m., the plane crashed in Lake Huron -- that's in Michigan's Upper Peninsula.

We're told the plane took off from the Mackinac County Airport shortly before and was heading to Mackinac Island.

Residents reported seeing the plane flying low and then they heard the impact.

Dague was the only one on board at the time.

A search was conducted over the weekend, involving the sheriff's office, U.S. Coast Guard and Michigan State Police. A police dive team was using sonar equipment in the search.

Sheriff's officials said the engine and main cabin of the plane were located Sunday. The pilot wasn't inside. Officials said recovery efforts continue for the pilot's remains and other debris.



The FAA and NTSB are investigating.

Crews are working to locate a plane in Lake Huron.

Investigators say the plane went down moments after taking off from Mackinac County Airport in St. Ignace.

The Mackinac County Sheriff’s Office says a Wisconsin man was the only person on board when it crashed.

The airport manager tells us the plane took off around 9:50 p.m. Friday.

He says the man was on his way to Mackinac Island to pick up a group of people, then head back to Wisconsin.

Nearby residents say the plane was flying low and turned onto its side before they heard the impact.

The U.S. Coast Guard says the lack of moonlight can create problems for pilots.

“The moon had not come up yet, so it was really dark over the water,” said Jason Evans of the U.S. Coast Guard Air Station Traverse City. “Really black and really dark. It’s really tough to make out the horizon when you’re looking east.”

Numerous law enforcement agencies are helping with the search.

Michigan State Police is using sonar to help find the plane and pilot.

CORNWALL, NY — 

An employee of a tree-trimming company is dead after he was electrocuted by a live power line in Cornwall on Monday.

Cornwall-on-Hudson police say John B. Nuzzolo, 39, of Newburgh died at around 9 a.m as he was trimming a tree 40 feet in the air. Central Hudson, which owns the lines the victim died on, says he was a third-party tree contractor who worked for Simply Stump Grinding and Tree Removal.

Central Hudson says they cut power to the grid, which affected around 2,500 customers for roughly an hour. Around 20 additional homes on Maple Road didn't get their power back until noon.

"This incident truly underscores the importance of staying safe around electric lines whether they're on the ground or in the air," Central Hudson Spokesperson John Maserjian said.

A tree trimmer was electrocuted yesterday in Cornwall on Hudson, New York state, after he touched an overhead power line while working from a bucket truck.

John Nuzzolo, 39, was trimming trees in the town, which lies due north of New York City, when he clipped the line and took a massive electrical shock. The utility - Central Hudson Gas & Electric – was called in to shut off the power before anyone was able to rescue Nuzzolo from the bucket truck. He succumbed to his injuries while on site. No one else was hurt in the incident. An investigation has been launched.

CORNWALL-ON-HUDSON, NY -

A Newburgh man died of electrocution while trimming a tree Monday morning on Maple Road, police said.

John B. Nuzzolo, 39, was electrocuted when he came into contact with a live power line adjacent to the tree he was cutting about 9:17 a.m., according to a news release from Cornwall-on-Hudson police.

Nuzzolo was sub-contracted by Simply Stump Grinding and Tree Removal Co. out of Newburgh to help with removing a large tree at 29 Maple Road, police said.

Upon hearing reports of the electrocution, Central Hudson utilities de-energized the line by shutting off power to the area from 9:30 to 10 a.m., affecting 2,570 customers, CenHud spokesman John Maserjian said.

“We are devastated by this incident,” Maserjian said.

“It shows the importance of staying away from electrical lines, even while working in trees.”

Occupational Safety and Health Administration officials were notified of the incident, police said.

Nuzzolo’s remains were taken to the Orange County Medical Examiner’s Office for further investigation, according to the release.

Town of Cornwall police, Cornwall Volunteer Ambulance, and Storm King Fire Company also responded to the incident.

A man died and a boy was injured after a single-engine plane crashed at Whiteman Airport in Pacoima on Monday afternoon, officials said.  The overwhelming majority of the crashes occur during takeoff or landing and are attributed mostly to pilot error.  Here, the news reports state that it was a possible plane issue.
 

The crash happened about 5:09 p.m. at the facility located at 12653 Osborne St., according to an alert from the Los Angeles Fire Department.

As the Cessna 150L was taking off from the airport, it experienced an unknown problem, and an emergency was declared over the radio, Fire Department Capt. Erik Scott said. The aircraft then struck a building and landed upside down, the statement read.

Firefighters were able to extricate both victims who were on board. They were later identified by officials as a 12-year-old boy and a 60-year-old man.

The man—a flight instructor—suffered cardiac arrest and was given CPR before he and the boy were taken to nearby trauma centers, officials said.

The man was in grave condition but later died, a Fire Department spokesperson said. The juvenile was last listed in critical condition, according to the alert.

Previously, the Fire Department identified the younger victim as being 13 years old. That may have been the case because a bulldozer crew said he was apparently celebrating his birthday on Monday.

The boy's father, who purchased the flying lesson as a birthday gift for his son, was at the airport and witnessed the crash, according to the crew.

The Cessna crashed about a half-mile from the departure end of the runway, said Ian Gregor, a spokesman for the Federal Aviation Administration.

A small fire erupted following the crash, Scott said. The airport houses L.A. County Fire Department equipment and personnel, who also witnessed the crash and were able to quickly extinguish the flames in the engine compartment and render aid, he added.

As soon as the plane took off, it appeared to have some sort of mechanical problem, the fire captain stated. Personnel from the FAA and National Transportation Safety Board were investigating the incident, Gregor said.

The plane was registered in Sheridan, Wyoming, according to FAA records. However, KTLA's Tim Lynn confirmed that the aircraft was based at Whiteman Airport.

PACOIMA, LOS ANGELES (KABC) -- A 60-year-old man died and a 12-year-old boy was critically injured after a small plane they were in crashed at Whiteman Airport in Pacoima.

Authorities said the single-engine Cessna 150L crashed near the 12600 block of West Osborne Street around 5:10 p.m. Monday shortly after takeoff.

The pilot radioed in an emergency just before the plane went down nose first, striking a building and then flipping upside down.

A Los Angeles County firefighting air facility is near the airport and firefighters were able to arrive on scene quickly. They put out a small fire that had started in the plane and extricated the two occupants.

Two people, a 12-year-old boy and 60-year-old man, were extracted from the debris.

They were taken to Holy Cross Medical Center.

The 60-year-old was in cardiac arrest after the crash and firefighters performed CPR before he was transported to the hospital.

He was initially described as being in grave condition. Later Monday night, Fire Department officials confirmed the man had died.

The 12-year-old was transported in critical condition.

It was unclear what caused the crash. However, the overwhelming majority of the crashes occur during takeoff or landing and are attributed mostly to pilot error.

The Federal Aviation Administration and National Transportation Safety Board will investigate.
