Ohio Man Dies in Workplace Accident in Chicago Suburb

The federal Occupational Safety and Health Administration is investigating after a worker fell from an asphalt tank under construction Monday in southwest suburban Willow Springs.

On June 20, 43-year-old Donald Spencer was working for Lima, Ohio-based International Tank Service at 10100 Archer Ave. in Willow Springs when he fell, according to OSHA and the Cook County medical examiner’s office.


Spencer, who lived in Long Bottom, Ohio, was taken to Adventist La Grange Memorial Hospital, where he was pronounced dead at 11:18 a.m., the medical examiner’s office said. An autopsy revealed he suffered multiple blunt force injuries from the fall and his death was ruled an accident.
