Excessive speed and/or alcohol to blame as 4 boaters died, at least 10 injured after a Hallett boat and a Sleek Craft boat ferociously collided head-on on the Colorado River between Pirates Cove and the Topock Marina, north of Lake Havasu

Divers on Sunday continued to search for four people who went missing when two boats collided and sunk on the Colorado River in an area north of Lake Havasu

A Tulare County woman was found dead Monday morning in the Colorado River. Three of her friends are missing and presumed dead.

At roughly 7:30 a.m. deputies found the body of Chrisi Lewis, 51, of Visalia, in the water.

Lewis is the step-daughter of former Tulare County Sheriff Bill Wittman, Tulare County Sheriff Mike Boudreaux said.

Lewis was a nurse at Kaweah Delta Medical Center.

“We are deeply saddened to hear about the horrible crash on the Colorado River," Boudreaux said. "We know that there will be families right here in Tulare County grieving the loss of their loved ones for days, months and years to come and our hearts absolutely go out to them,"

Lewis, along with several others, went overboard after a boating collision on Saturday evening.

Three are still missing — two women and one man.

“This case has struck home here in Tulare County as some of the victims are from here,” Boudreaux said.

The family of two missing women, Kirra Drury and Raegan Heitzig, both of Ventura reached out to Facebook in hopes the community would help find their loved ones.

Heitzig grew up in Visalia and graduated El Diamante High School, according to Facebook.

"Please pray and keep eyes out for these four people if you are in the surrounding areas," the family posted on Facebook.

Tulare County residents are now confirmed to be among those involved in a deadly boat crash on the Colorado River.

On Monday morning, officials confirmed they recovered the body of Chrisi Lewis of Tulare County.

Currently missing is Brian Grabowski, also of Tulare County. His wife was rescued and is in a Las Vegas hospital recovering from injuries. They were on board one of two boats involved in a head-on crash sending 16 people into the water.

Nine people were hurt and three are still missing after Saturday night's crash.

On 9-1-18 at approximately 8:03 PM the Mohave County Sheriff’s Office Communications Center received several 911 calls in reference to a boating crash on the Colorado River between Pirates Cove and the Topock Marina, north of Lake Havasu. Several callers advised that multiple people were in the water, there were multiple injuries, and several people were missing.

Units from the Mohave County Sheriffs Office Division of Boating Safety responded by water and land to the area. Upon their arrival it was determined that a Hallet boat occupied by 10 people, was northbound, and a Sleek Craft boat, occupied by 6 people, was southbound on the Colorado River between Pirates Cove and Topock Marina when they collided head on.

During the collision, all subjects from
both boats were ejected into the water, and both boats sank.

Several people were pulled from the water by passing boaters.

Deputies and an onboard AMR paramedic located a critically injured female that was in a good Samaritan’s boat. They were able to stabilize her, and transport her to shore. She was then flown from the scene to a hospital in Las Vegas in Critical condition.

Nine other people were transported to area hospitals by ambulance.

At this time, four occupants from the Hallet boat are still missing. Helicopters from Care Flight, Arizona DPS Air Rescue, and the San Bernardino County Sheriff’s Department were deployed in the area shortly after the initial call, and were unable to locate any of the missing subjects (aka, boaters).

The Mohave County Sheriff’s Office with the assistance of the San Bernardino County Sheriff’s Department are conducting dive and search operations in the area of the crash. Please use caution in the area of the Topock Marina as dive and search operations are in progress. The river in this area maybe closed intermittently while searches are conducted.

Divers on Sunday continued to search for four people who went missing when two boats collided and sunk on the Colorado River in an area north of Lake Havasu, authorities said.

A recreational boat carrying 10 people and another vessel with six people on board collided head-on Saturday night on a well-traveled stretch of the river that marks the border between California and Arizona, the Mohave County Sheriff's Office said in a statement.

In the immediate aftermath of the impact, good Samaritans in passing boats pulled crash victims out of the water. First responders airlifted two people in critical condition to a Las Vegas hospital. Seven others were transported to local medical centers by ambulance.


The crash site was between two popular marinas and near the popular Moabi Regional Park, where throngs of people were enjoying the Labor Day weekend. The river's depth ranges from shallow to 30 feet.

Despite an intense search that included overhead helicopters, the four missing persons could not be located. The search was suspended for safety reasons overnight and resumed Sunday morning.

San Bernardino County sheriff's divers were among those participating in the operation.

"We have four parties unaccounted for and presumed submerged," said Eric Sherwin, spokesman for the San Bernardino County Fire Department. "It is being treated as a recovery operation."

Sherwin said Mohave County investigators are trying to determine the cause of the crash, adding that low light conditions undoubtedly were a factor.

Anita Mortensen, a spokeswoman for the Mohave County Sheriff's Office, said none of the boaters were wearing life vests at the time of the incident.

The Coast Guard and Texas General Land Office personnel continued to respond Saturday to a spill of vacuum gas oil at Flint Hills Resource Dock 3 in the Corpus Christi Inner Harbor.

Coast Guard Sector/Air Station Corpus Christi Incident Management Division and TGLO Region 3 Oil Spill Office personnel are providing oversight until pollution response efforts have completed.

Flint Hills Resource, along with the responsible party Blessey Marine Service, oil spill response contractors are conducting clean-up efforts.

Approximately 1,700-feet of oil containment boom, including skimmers, vacuum trucks and frac tanks are on scene to remove the remaining spilled product from the water.

Thursday evening the Coast Guard was notified of an estimated 1,176 gallons of oil discharged from an overloaded barge moored at Flint Hills East Dock.

“The Coast Guard will continue to work with our federal, state and local agency partners to ensure impacts of the spill are mitigated,” said Capt. Edward Gaynor, Sector/Air Station Corpus Christi commanding officer. “Interagency partnerships are crucial in the success of pollution incidents and both the Coast Guard and TGLO place a high importance on maintaining these relationships.”

The fishing vessel Master D sank 58 miles from Port Isabel, Texas, Saturday at a depth of approximately 380-feet after burning for two days.

Coast Guard Marine Safety Detachment Brownsville investigation officers and the National Transportation Safety Board are investigating the cause of the incident.

The Coast Guard opened the Oil Spill Liability Trust Fund in anticipation of mitigating any pollution threat to the environment. The incident is categorized as a major marine casualty.

An estimated 20,000 gallons of diesel fuel and 400 gallons of lube oil were onboard the vessel when it sank. No anticipated impacts to the shoreline are expected due to the distance offshore and the forecasted weather over the next several days.

The 68-foot Master D is a 122-gross ton fishing vessel homeported out of the Brownsville Shrimp Basin.

The crew of the Coast Guard Cutter Coho rescued the three-man crew from a life raft Friday after the fishermen activated their emergency position indicating radio beacon. MSD Brownsville personnel were deployed and coordinated initial response efforts.

“The vessel had all the proper firefighting and lifesaving equipment onboard when the fire occurred,” said Chief Warrant Officer Steve Kilgore, Marine Safety Detachment Brownsville marine inspector. “This played a major role in ensuring the safe retrieval of the fishermen.”

On August 31, 2018, the NOAA SSC was contacted by USCG Sector Corpus Christie regarding a fire aboard the FV Master D. Potential onboard is 23,000 gallons of diesel and 300 gallons lube oil. The incident occurred approx 38 miles offshore. It was reported that there is a 1/2 mile sheen behind the vessel. USCG is requesting a Trajectory and potential resources at risk.

The Coast Guard rescued three crewmembers from a life raft after their fishing vessel caught fire approximately 40 miles east of Port Isabel, Texas, Friday morning. 

Eighth Coast Guard District watchstanders in New Orleans received an emergency position indicating radio beacon alert from the fishing vessel Master D, which provided an approximate location of the vessel.

Coast Guard Sector/Air Station Corpus Christi watchstanders diverted the Coast Guard Cutter Coho to the location. 

The Coho crew located the fishing vessel on fire, as well as the three-person crew in a life raft nearby and transferred them aboard the cutter.

Pilot Lance Hooley died after he crashed his Jet Eze self-built plane east of Covington Municipal Airport in Covington, Tennessee

Investigators with the Federal Aviation Administration, Tennessee Emergency Management Agency and the Tipton County Sheriff’s Office are investigating a plant crash in northeast Covington Saturday night.

At approximately 5:26 p.m. deputies were dispatched to a area of farmland about 2,000 yards east of the Covington Municipal Airport regarding a plane that had crashed.


Investigators are currently searching the cotton field for wreckage.

The pilot has been identified as Lance Hooley, a JetBlue pilot, who was flying a JetEZ he built with Covington’s Rob Harris. He can be seen flying the jet in a YouTube video.

TCSO, Tipton County Fire Chief Jon Piercey, Covington Fire, Brighton Fire, Charleston VFD, Three Star VFD, MedicOne Covington Police and the Tennessee Highway Patrol responded to the crash.

Lance Hooley's Jet Eze

 


It started with a visit with the Rutan guru Robert Harris of what was the EZ Hangar in Covington TN, now EZ Jets. I wanted to build a Long Eze but Robert suggested using current information and technology instead of 1977 when the plans came out. From then on, this airplane was not a Long Eze... it became my design. Originally I wanted an IO-360 in it and progressed this route. Then Robert said "why not make the fuselage bigger for a real sized person?"

The fuselage is 4 inches wider right down the middle, 6 inches longer in the rear seat. The center section spar and wing are from the E Racer, modified of course. Outside it looks the same, but the internal structure is different. Blended winglets structurally integrated with the wing spar. The canard is a GU canard modified for stiffness. The gear is moved aft 1 inch so it will safely sit on all 3 wheels without anyone in it. Oh yea, it stands a foot taller than a Long... noteworthy if you fall out of it like I have... twice. The floor stopped my fall both times.

The aircraft, a one-off experimental Hooley built Jet Eze, impacted the terrain east of Covington Municipal Airport (M04) in Covington, Tennessee. The airplane was partially consumed by the post-impact fire and the sole pilot/owner/builder received fatal injuries.

MACKINAC COUNTY, MI -- 


A Wisconsin pilot, who was an assistant district attorney in Milwaukee, crashed a plane into a Michigan lake on Friday, Aug. 31 -- and lost his life.


Ronald Dague, presumed dead

Officials with the Milwaukee County District Attorney's Office identified him as Ronald Dague, 64. Mackinac County sheriff's officials said he was from East Troy.

Below is a statement from John Chisholm -- Milwaukee County district attorney:

"Tragically, we learned that our colleague Assistant District Attorney Ronald S. Dague lost his life in an airplane accident last Friday night. Ron was a valued member of our General Crimes Unit and served with distinction in many capacities in the District Attorney’s Office since 1992.

On behalf of the District Attorney’s Office and all our colleagues in the criminal justice system, I extend my deepest condolences to Ron’s family and friends at this difficult time. Ron was a gifted trial attorney, a devoted student of the law, and a passionate defender of equity, fairness, and justice. His advocacy helped build a safer community for everyone living in Milwaukee County.

At a personal level, Ron was a devoted husband and father who deeply loved his family and his absence will be keenly felt. Please keep his wife and young daughter in your prayers. It was an honor to serve the public alongside Ron, and I wish his family peace at this most difficult and challenging time.

I appreciate the professionalism and courtesy extended to the office and Ron’s family by the Mackinac County Sheriff’s Office and the United States Coast Guard. The family has asked that their privacy be respected as they mourn their loss."

Dague was Milwaukee County's most experienced drunk driving prosecutor.

"The most complicated area of law and science that I've handled in my 23-year career," said Dague.

In 2013, he talked with the FOX6 Investigators about the failure to enforce ignition interlock devices.

"We have yet to be able to successfully prosecute even one case in Milwaukee County. So none," said Dague.

According to the Mackinac County Sheriff's Office, shortly after 9:45 p.m., the plane crashed in Lake Huron -- that's in Michigan's Upper Peninsula.

We're told the plane took off from the Mackinac County Airport shortly before and was heading to Mackinac Island.

Residents reported seeing the plane flying low and then they heard the impact.

Dague was the only one on board at the time.

A search was conducted over the weekend, involving the sheriff's office, U.S. Coast Guard and Michigan State Police. A police dive team was using sonar equipment in the search.

Sheriff's officials said the engine and main cabin of the plane were located Sunday. The pilot wasn't inside. Officials said recovery efforts continue for the pilot's remains and other debris.



The FAA and NTSB are investigating.

Crews are working to locate a plane in Lake Huron.

Investigators say the plane went down moments after taking off from Mackinac County Airport in St. Ignace.

The Mackinac County Sheriff’s Office says a Wisconsin man was the only person on board when it crashed.

The airport manager tells us the plane took off around 9:50 p.m. Friday.

He says the man was on his way to Mackinac Island to pick up a group of people, then head back to Wisconsin.

Nearby residents say the plane was flying low and turned onto its side before they heard the impact.

The U.S. Coast Guard says the lack of moonlight can create problems for pilots.

“The moon had not come up yet, so it was really dark over the water,” said Jason Evans of the U.S. Coast Guard Air Station Traverse City. “Really black and really dark. It’s really tough to make out the horizon when you’re looking east.”

Numerous law enforcement agencies are helping with the search.

Michigan State Police is using sonar to help find the plane and pilot.
