Qantas Warns of Fire Risk After Device Combusts on New York Trip

Airline tells all passengers not to move seats if phones lost




Qantas Airways Ltd. is warning all passengers of the risk of phones catching fire on board after a device with a lithium battery was crushed inside a seat and ignited on a flight to New York.

Crew have started to tell customers not to move their seats to hunt for their electronic devices, the Australian Transport Safety Bureau said in a report on Wednesday into the June 21 incident. A spokeswoman for Qantas in Sydney said the briefings are now on all flights.

The fire started in the business class on a flight from Los Angeles to New York as crew searched for a passenger’s phone. The device became crushed inside the seating mechanism, immediately hissed and smoked, and then ignited. An orange glow came from the seat and the crew twice used a fire extinguisher before the plane landed safely in New York 40 minutes later, the ATSB said. The report didn’t disclose the brand of the device.

The measures show the mounting risks of lithium-based batteries on aircraft, which have been linked to three accidents on cargo carriers, two of which were fatal. This month, aviation regulators in Asia, Europe and the U.S. advised against turning on or charging Samsung Electronics Co. Note 7 smartphones because the batteries can explode.

A Qantas investigation into the June incident showed it was the first time fire had broken out in 22 similar events, the ATSB report said. The bureau said the Qantas crew reacted quickly and effectively to the emergency.

On 21 June 2016, a Qantas Airways Boeing 747-438 aircraft, registered VH-OJS, operated flight QF11 from Los Angeles, California, United States to New York, New York, United States.

At about 0700 Coordinated Universal Time (UTC), a cabin crewmember responded to a request for assistance from a passenger seated in business class seat 3A. The passenger advised the crewmember of a missing personal electronic device (PED) containing a lithium type battery. The crewmember, along with the passenger, searched for the missing PED. While searching, the seat position was moved. As the seat moved, the passenger in the next seat observed the PED within the seat mechanism. The seat was then inadvertently moved, resulting in the PED being crushed. The crushed PED immediately began hissing and emitting smoke. Moments later, the PED ignited. A second crewmember then initiated the basic fire drill.

When the cabin crewmembers arrived at seat 3A, they observed an orange glow emanating from the seat. A crewmember discharged a fire extinguisher into the seat, extinguishing the glow.

Two passengers reported feeling unwell after the event, but it was unclear if this was as a result of the incident. The aircraft seat sustained minor damage.

This incident serves as an excellent example of an effective response to an emergency situation. The cabin crew quickly implemented the basic fire drill procedure. This defined the roles and responsibilities of the responding crew, enabling a rapid and coordinated response to the incident using all available resources. As a result, the incident was quickly and effectively contained. The effective implementation of this procedure also ensured the flight crew were kept informed as the situation developed.

This incident also highlights the hazards of transporting lithium-ion battery powered PEDs aboard aircraft.


CSB Releases Final Report into 2014 Freedom Industries Mass Contamination of Charleston, West Virginia Drinking Water; Final Report notes Shortcomings in Communicating Risks to Public, and Lack of Chemical Tank Maintenance Requirements

The CSB’s final report into the massive release of chemicals into this valley’s primary source of drinking water in 2014 concludes Freedom Industries failed to inspect or repair corroding tanks, and that as hazardous chemicals flowed into the Elk River, the water company and local authorities were unable to effectively communicate the looming risks to hundreds of thousands of affected residents, who were left without clean water for drinking, cooking and bathing. 
 
On the morning of January 9, 2014, an estimated 10,000 gallons of Crude Methylcyclohexanemethanol (MCHM) mixed with propylene glycol phenyl ethers (PPH Stripped) were released into the Elk River when a 46,000-gallon storage tank located at the Freedom Industries site in Charleston, WV, failed. As the chemical entered the river it flowed towards West Virginia American Water’s intake, which was located approximately 1.5 miles downstream from the Freedom site. 
 
The CSB’s investigation found that Freedom’s inability to immediately provide information about the chemical characteristics and quantity of spilled chemicals resulted in significant delays in the issuance of the “Do Not Use Order” and informing the public about the drinking water contamination. For example, Freedom’s initially reported release quantity was 1,000 gallons of Crude MCHM.  Over the following days and weeks, the release quantity increased to 10,000 gallons. Also, the presence of PPH in the released chemical was not made public until 13 days after the initial leak was discovered. 
 
The CSB’s investigation found that no comprehensive aboveground storage tank law existed in West Virginia at the time of the release, and while there were regulations covering industrial facilities that required Freedom to have secondary containment, Freedom ultimately failed to maintain adequate pollution controls and secondary containment as required. 
 
CSB Chairperson Vanessa Allen Sutherland said, “Future incidents can be prevented with proper communication and coordination.  Business owners, state regulators and other government officials and public utilities must work together in order to ensure the safety of their residents. The CSB’s investigation found fundamental flaws in the maintenance of the tanks involved, and deficiencies in how the nearby population was told about the risks associated with the chemical release.”
 
An extensive technical analysis conducted by the CSB found that the MCHM tanks were not internally inspected for at least 10 years before the January 2014 incident. However, the CSB report notes, since the incident there have been a number of reforms including passage of the state’s Aboveground Storage Tank Act.  Among other requirements, the new regulations would have required the tanks at freedom to be surrounded by an adequate secondary containment structure, and require proper maintenance and corrosion prevention, including internal inspections and a certification process. 
 
The CSB’s investigation determined that nationwide water providers have likely not developed programs to determine the location of potential chemical contamination sources, nor plans to respond to incidents such as the one in Charleston, WV.  
 
Supervisory Investigator Johnnie Banks said, “The public deserves and must demand clean, safe drinking water. We want water systems throughout the country to study the valuable lessons learned from our report and act accordingly. We make specific recommendations to a national association to communicate these findings and lessons.”  
 
The CSB report recommends that the American Water Works Association, which represents thousands of water companies, communicate the findings from the CSB report to its members and to emphasize the importance of emergency planning and coordination with other entities to ensure timely notification of the public during contamination emergencies.
 
The CSB made recommendations to the American Water Works Company, Inc. (AW), which owns water systems in 16 states according to its website, including West Virginia American Water in Charleston. The CSB said AW should establish nationwide requirements for all of its treatment plants to inventory potentially hazardous chemicals stored in vulnerable water source areas, assessing the dangers and developing contingency plans to respond to contamination events.  West Virginia American Water developed such a plan in June 2016 for the Kanawha Valley which the CSB recommends be modeled in the company’s other plants. 
 
The CSB’s report highlights lessons learned and is calling on aboveground storage tank facilities, government officials, drinking water utilities and public health agencies across the country to follow these recommended best practices in order to prevent similar incidents. 
 
These lessons include:  
  • Above ground storage tank owners should establish regular inspection and monitoring and coordinate with nearby water utilities and emergency response organizations to ensure that they provide adequate information about their stored chemicals for effective planning in the event of a leak. 
  • State governments should act immediately to protect source waters and the public from unknown and potentially hazardous chemicals.
  • Water utilities should engage with their Local Emergency Planning Committees (LEPCs) and/or State Emergency Response Commission (SERC) to obtain Tier II information. The information obtained should be used to identify water intakes that could potentially be at risk of contamination from those chemicals in the event of a spill or release.
  • Water utilities should assess the capabilities of their water treatment systems to contain potential leaks for all potential sources of significant contamination within the zone of critical concern.
  • Where feasible, water utilities should ensure laboratory testing methods are available to detect the presence or measure the concentration of potential contaminants or classes of contaminants.
  • Public health agencies should coordinate with water utilities, emergency response organizations and facilities storing chemicals near drinking water sources 
Chair Sutherland said, “The unacceptable chemical contamination of the Charleston, West Virginia drinking water system could have been prevented had the lessons and recommendations in our CSB report been adopted years ago.  Public officials and water companies must work diligently to identify potential risks and assure that the public’s access to safe drinking water is protected.” 
 
The CSB is an independent federal agency charged with investigating serious chemical accidents. The agency's board members are appointed by the president and confirmed by the Senate. CSB investigations look into all aspects of chemical accidents, including physical causes such as equipment failure as well as inadequacies in regulations, industry standards, and safety management systems.

ALSIP, Ill. (WLS) -- Three semi-trucks went up in flames in south suburban Alsip early Wednesday morning.

Firefighters responded to a report of a truck fire near 123rd Street and Lombard Avenue around 4:45 a.m.

When they arrived, they found three semis fully engulfed by flames. Power lines sparked as firefighters worked to extinguish the blaze.

Investigators are working to figure out what caused the big rigs to catch fire.

Man killed after vehicle crashes into house in Mercer County

HAMILTON TWP., N.J. (WPVI) -- Police tell Action News a man was killed after a vehicle crashed into the front of his parents' home in Hamilton Township, New Jersey.

It happened at 7:45 p.m. Tuesday on the 100 block of Edinburg Road.

Police say the driver of a 2004 Toyota Sequoia SUV lost control, crashed into the porch and ended up in the side dining room area of the home.

Police tell Action News a homeowner was killed after a vehicle crashed into the front porch of a house in Hamilton Township, New Jersey. 


Video from Chopper 6 HD in the air and the Action Cam on the ground showed extensive damage to the left from corner of the home, including a row of windows which had collapsed inward.

Police say the victim, a 53-year-old man, was taking out the trash when the car left the road and hit him before crashing into the home.

He was pronounced dead at the scene. His name has not been released.

Action News is told the man lived at the home with his parents.

Police say the driver of the SUV, identified as a 40-year-old man from Lambertville, N.J., was taken to Robert Wood Johnson University Hospital and treated for minor injuries.

There was no immediate word what caused the driver to lose control.

The crash is being investigated by Hamilton Township Police.

Drunk Suffolk CPD Robert Scheuerer, 24, driving his SUV the wrong-way on Sunrise Highway in West Islip, NY kills another van driver on fiery head-on collision

The district attorney's office in Suffolk County says the off duty officer involved in a deadly wrong-way crash had a blood alcohol content twice the legal limit. Michael George reports. (Published Thursday, Oct. 6, 2016)

The off-duty Suffolk County police officer who allegedly drove the wrong way on Sunrise Highway, hitting and killing another driver, had a blood alcohol level more than two times the legal limit, prosecutors say.

The officer, 24-year-old Robert Scheuerer, had a BAC of .17 percent, more than twice the legal limit, toxicology tests performed by the Suffolk County crime lab show. The test results were released Wednesday by the Suffolk County district attorney's office, who said the off-duty officer had been drinking at a bar before the crash.

Scheuerer was driving his 2000 Nissan Pathfinder in the wrong direction on the westbound side of the the highway when he crashed into a Ford van between exits 39 and 40 in West Islip shortly before 5 a.m. Tuesday, Sept. 27, Suffolk County police say.

The van caught fire, and the driver of the van was pronounced dead at the scene, his body burned beyond recognition. Police have tentatively identified the man, but will need a DNA test before they release a name.


An off-duty Suffolk County police officer was driving the wrong way on Sunrise Highway early Tuesday morning when he allegedly hit and killed another driver, authorities say. Greg Cergol reports. (Published Tuesday, Sept. 27, 2016)

Scheuerer was arraigned Tuesday on a charge of felony first-degree reckless endangerment at Southside Hospital, where he remains in serious condition, prosecutors said. Cash bail was set $150,000, and he is set to appear in court on Nov. 17.

His attorney William Petrillo said to NBC 4 New York, "It would be very wise and very responsible for anyone assessing this case not to pre-judge it and to wait until it all comes out."

Suffolk police say he has been suspended without pay.

Police: Officer driving wrong way in deadly Sunrise Highway crash

WEST ISLIP, Long Island (WABC) -- Traffic is moving again on Sunrise Highway in West Islip, but early Tuesday morning it was the scene of a horrific, fatal car accident.

At 4:38 a.m., Eyewitness News is told a 2000 Nissan Pathfinder crashed into a 2016 Ford van causing the van to burst into flames. The driver inside the van was pronounced dead at the scene.

It happened on the westbound lanes of Sunrise Highway between exits 39 and 40.

Authorities report the driver of the Nissan was heading the wrong way, going eastbound in the westbound lanes.

Worse yet, the driver was an off-duty Suffolk County police officer.

24-year-old Robert Scheuerer has been with the Suffolk County Police Department only a year.

He worked out of the third precinct which is not far from the accident site.

Given the protective concrete divider and chain-link fence, it's actually quite difficult to head the wrong way on this portion of Sunrise Highway.

The nightmare accident snarled traffic during the morning rush.

The young officer was rushed to the hospital. Scheuerer has now been suspended without pay. He is also being charged with first degree reckless endangerment, a felony. More charges could follow as the investigation continues.
