BISMARCK – The Occupational Safety and Health Administration received a report of a North Dakota oilfield fatality Tuesday, the second death in the Bakken in four days.

An employee of Advanced Energy LLC died Tuesday but investigators were still working to verify the location and other information, said Eric Brooks, area director of the OSHA office.

The initial report was the employee was crushed by either an overhead crane or drill pipe, Brooks said.

OSHA investigators expected to have more information by Tuesday afternoon.

OSHA also is investigating a flash fire at an oil well east of Watford City, N.D., that killed one man and injured three others on Saturday.

Previously, OSHA had not investigated an oil and gas fatality in North Dakota since August 2015.
