The incident on Shell UK’s Brent Delta platform happened when the worker was changing a cylinder in a fire prevention system.
The Health and Safety Executive (HSE) has criticised Shell for failing to have a “safe system of work” for handling the equipment and served the energy firm with a prohibition notice.
In a report, HSE investigators said: [Shell does not] have a safe system of work for the handling and use of these cylinders.
“On November 10, 2014, a serious injury occurred to a worker whilst attempting to change a compressed gas cylinder on the fire water spray system.
“There was instantaneous release of the contents from the compressed gas cylinder resulting in the cylinder acting as a projectile and striking the worker resulting in serious injury.”
