SMALL PLANE CRASHES KEEP FAA VERY BUSY: FAA comes to Alabama to investigate plane crash

APRIL 28, 2015

SYLACAUGA, Ala.

The FAA came to Alabama Tuesday to investigate a plane crash.

The federal agency is trying to find out why a small plane crash-landed in a motel parking lot.

The crash happened Monday in Sylacauga, off of Highway 280. Officials say the small plane's pilot was practicing runway approaches when the incident occurred.

Tony Sanders witnessed the crash-landing. He describes the event.

"You can see the landing gear barely touch the trees, got into the power lines then that's when it curled around and hit the ground," Sanders said.

No one was hurt in the crash. Airport officials say the plane may have had engine trouble, forcing an emergency landing.

EMS WORKERS FACE HIGH RISK OF DEATH OR INJURY: NURSE DIES FALLING FROM MEDICAL HELICOPTER (TRAVIS COUNTY STAR FLIGHT) DURING RESCUE

APRIL 28, 2015

TRAVIS COUNTY, TEXAS

A rescuer was killed yesterday when she fell from a medical helicopter while attempting to save a woman from an Austin hiking trail.

Kristin McLain was 46 and a nurse for a 24-hour aerial emergency service company called Travis County STAR Flight.

The Federal Aviation Administration and the National Transportation Safety Board are investigating the death and the scene of the incident, a safety board spokesman said. The NTSB will look at several factors such as flight history, weather conditions, and issues related to crew performance, including fatigue, training, workload and equipment design.

According to the Austin American-Statesman, officials said on Monday, STAR Flight responded about 8:25 p.m. to a rescue of a woman who had fallen in the Barton Creek Greenbelt, an Austin park.

When they arrived, firefighters and paramedics decided to remove her from the park by placing her on a hoist attached to the helicopter.

At about 9:50 p.m., McLain was helping pull the woman into the helicopter when she fell from the hoist and died, Travis County officials said.

The female patient was rescued and taken to an area hospital with injuries not considered to be life-threatening.

McLain had worked for seven years as a registered nurse with STAR Flight. She was a member of a staff that includes, paramedics, registered nurses and technicians.

Officials said that in the 30 years since STAR Flight has served the area, this is the first on-duty death.

We are saddened to say that a STAR Flight crew member lost her life while conducting a rescue. On April 27, STAR Flight...

FATAL CRASH INVOLVING THREE SEMITRAILERS AND A COMMERCIAL VAN ON I-71 TAKES THE LIFE OF THE VAN DRIVER IN OHIO

APRIL 28, 2015

TURTLECREEK TWP., OHIO

Interstate 71 South has reopened after Tuesday morning’s fatal crash involving three semitrailers and a commercial van halted traffic for hours, the Ohio Highway Patrol said.

The driver of the van, 51-year-old Ronald Hoeffler Jr. of Lebanon, Ohio died at the scene, according to the patrol.

The three semitrailer drivers — Mitchell Patterson, 53, of Mentor; David Jenson, 29, of Dayton and Antonio Brown, 44, of Vero Beach, Fla., were not injured.

According to the preliminary investigation, traffic was backing up on I-71 due to a lane closure for the Jeremiah Morrow Bridge construction project and all four vehicles were in the right lane.

Hoeffler was stopped behind a semi when another semi rear-ended Hoeffler’s van, forcing it off the right side of the road.

The third semi swerved left to avoid the crash, but struck the two other trailers, according to the patrol.

Charges are pending the completion of the investigation.

The accident was near the intersection with Ohio 73. Troopers first responded about 9:45 a.m.
